摘要
目的研究基于营养风险筛查下的五阶梯营养方案对结直肠癌术后辅助化疗患者营养状态及肿瘤学指标的影响。方法选取2019年9月-2020年9月我科114例结直肠癌术后辅助化疗(有营养风险)患者作为研究对象,采用随机数字表法分为对照组和观察组,各57例。对照组给予常规饮食指导,观察组给予基于营养风险筛查下的五阶梯营养方案。比较两组营养状态、肿瘤学指标、手术部位感染率。结果观察组干预后清蛋白(ALB)、前清蛋白(PA)、总蛋白(TP)、转铁蛋白(TRF)水平均高于对照组(P<0.05);观察组干预后血清内癌胚抗原(CEA)、糖链抗原199(Ca199)水平均低于对照组(P<0.05);两组手术部位感染率比较差异无统计学意义(P>0.05)。结论营养风险筛查下的五阶梯营养方案对结直肠癌术后辅助化疗患者效果确切,能有效改善患者营养状态,降低肿瘤学指标,可应用于临床。
Objective To study the effect of five-step nutrition scheme under nutritional risk screening on nutritional status and oncological indicators in patients with colorectal cancer after adjuvant chemotherapy.Methods Selected 114 patients with colorectal cancer postoperative adjuvant chemotherapy(with nutritional risk)from September 2019 to September 2020 in our hospital as the research objects,and were divided into groups by random number table method,and 57 patients in the control group were given regular diet Instructed,57 patients in the observation group were given a five-step nutritional program based on nutritional risk screening,and the nutritional status,oncological indicators,and surgical site infection rates were compared between the two groups.Results After intervention,the levels of albumin(ALB),prealbumin(PA),total protein(TP)and transferrin(TF)in the observation group were higher than those in the control group(P<0.05).The levels of carcinoembryonic antigen(CEA)and carbohydrate antigen 199(Ca199)were lower than those in the control group(P<0.05);there was no significant difference in the infection rate of surgical site between the two groups(P>0.05).Conclusion The five-step nutrition scheme under nutritional risk screening is effective for colorectal cancer patients with postoperative adjuvant chemotherapy,which can effectively improve the nutritional status of patients,reduce oncological indicators,and has clinical promotion value.
