增强CT定量参数对胰腺癌和坏死性胰腺炎的鉴别诊断价值

Value of enhanced CT quantitative parameters in the differential diagnosis of pancreatic cancer and necrotizing pancreatitis

摘要 目的分析增强CT定量参数对胰腺癌和坏死性胰腺炎的鉴别诊断价值。方法抽取2018年2月至2021年2月南阳市第一人民医院收治的43例胰腺癌患者为胰腺癌组,并抽取医院同期收治的41例坏死性胰腺炎患者为胰腺炎组,两组均行增强CT定量参数诊断。比较两组胰腺增强CT值、增殖相关基因mRNA表达量、侵袭相关基因mRNA表达量,并分析相关性。结果胰腺癌组胰腺增强CT值(58.31±5.97)低于胰腺炎组(111.59±13.65),P<0.05。胰腺癌组RCC-32 mRNA表达量低于胰腺炎组,FOXC1、Ezrin、Bmi-1 mRNA表达量高于胰腺炎组(P<0.05)。胰腺癌组TFPI-2、Let-7f、FHL1 mRNA表达量低于胰腺炎组,Twist mRNA表达量高于胰腺炎组(P<0.05)。Pearson分析结果显示,胰腺癌患者胰腺增强CT值与增殖相关基因侵袭相关基因RGC-32(r=0.571)mRNA表达量呈正相关,与侵袭相关基因TFPI-2(r=0.643)、Let-7f(r=0.594)、FHL1(r=0.673)mRNA表达量呈正相关(P<0.05);与增殖相关基因侵袭相关基因FOXC1(r=-0.603)、Ezrin(r=-0.637)、Bmi-1(r=-0.524)mRNA表达量呈负相关,与侵袭相关基因Twist(r=-0.628)mRNA表达量呈负相关(P<0.05)。结论与坏死性胰腺炎患者相比,胰腺癌患者组织增强CT值较低,并与增殖、侵袭基因表达显著相关,可为临床早期诊疗提供依据。 Objective To analyze the application value of enhanced CT quantitative parameters in the differential diagnosis of pancreatic cancer and necrotizing pancreatitis.Methods Forty-three patients with pancreatic cancer treated in Nanyang First People’s Hospital from February 2018 to February 2021 were selected as the pancreatic cancer group,and 41 patients with necrotizing pancreatitis treated in the hospital during the same period were selected as the pancreatitis group.All of them were diagnosed with enhanced CT quantitative parameters.The enhanced CT value of pancreas,mRNA expression levels of proliferation-related genes and invasion-related genes were compared between the two groups,and the correlations among them were analyzed.Results The enhanced CT value of pancreatic cancer group was 58.31±5.97,lower than 111.59±13.65 of pancreatitis group(P<0.05).RCC-32 in pancreatic cancer group was lower than that in pancreatitis group,while mRNA expression of FOXC1,Ezrin and Bmi-1 were higher than those in pancreatitis group(P<0.05).mRNA expression of TFPI-2,Let-7f and FHL1 in pancreatic cancer group were lower than those in pancreatitis group,while mRNA expression of Twist was higher than that in pancreatitis group(P<0.05).Results of Pearson analysis showed that the enhanced CT value was positively correlated with mRNA expression level of proliferation-related gene RGC-32(r=0.571),positively correlated with mRNA expression levels of invasion-related genes TFPI-2(r=0.643),Let-7f(r=0.594),and FHL1(r=0.673);it was negatively correlated with mRNA expression levels of proliferation-related genes FOXC1(r=-0.603),Ezrin(r=-0.637),Bmi-1(r=-0.524),and negatively correlated with mRNA expression level of invasion-related gene Twist(r=-0.628),P<0.05.Conclusions Compared with patients with necrotizing pancreatitis,enhanced CT values of tissue in patients with pancreatic cancer is lower,which is significantly related to the expression of proliferation and invasion genes,therefore,it can provide evidence for early clinical diagnosis and treatment.
