慢性心力衰竭心功能分级与血清和肽素及脑钠肽的相关性分析 被引量:18

摘要 目的探讨慢性心力衰竭(chronic heart failure,CHF)患者心功能分级与血清和肽素(copeptin,CPT)及脑钠肽(brain natriuretic peptide,BNP)水平的相关性。方法回顾性分析山东省立第三医院2019年1月至2021年1月收治的150例CHF患者(CHF组)的临床资料,另选取同期心脏功能正常的150名正常体检者设为对照组。比较两组研究对象及不同心功能分级患者的血清CPT、BNP水平,采用受试者工作特征(receiver operating characteristic,ROC)曲线分析CPT、BNP水平预测CHF患者心功能分级的价值,采用Spearman系数分析CHF患者心功能分级与血清CPT、BNP水平的相关性。结果CHF组血清CPT、BNP水平显著高于对照组,差异有显著性(P<0.05)。心功能Ⅳ级患者血清CPT、BNP水平显著高于心功能Ⅱ、Ⅲ级患者,差异有显著性(P<0.05)。CPT、BNP预测心功能Ⅱ级的ROC曲线下面积分别为0.902、0.826,预测心功能Ⅲ级的ROC曲线下面积分别为0.653、0.690,预测心功能Ⅳ级的ROC曲线下面积分别为0.701、0.685。CHF患者心功能分级与血清CPT、BNP水平呈正相关性(r=0.396、0.327,P=0.000、0.000)。结论CHF患者心功能分级与血清CPT、BNP水平呈正相关,临床应当给予重视。 Objective To investigate the correlation between cardiac function classification and serum and peptide(CPT)and brain natriuretic peptide(BNP)levels in patients with chronic heart failure(CHF).Method The clinical data of 150 patients with CHF admitted to Shandong Provincial Third Hospital from January 2019 to January 2021 were retrospectively analyzed.They were set as CHF group,and 150 normal subjects who underwent physical examination and had normal cardiac function during the same period were selected as control group.Compare the CPT and BNP levels of the two groups of subjects with the CPT and BNP levels of patients with different cardiac function grades,use the ROC curve to analyze the value of CPT and BNP levels to predict the cardiac function grade of CHF patients,and finally use the Spearman coefficient to evaluate the cardiac function grade and serum CPT of CHF patients.Correlation analysis of BNP level.Result The CPT and BNP levels in the CHF group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of CPT and BNP in patients with grade Ⅳ cardiac function were significantly higher than those with grade Ⅱ and Ⅲ cardiac function(P<0.05).According to ROC analysis,cardiac function level Ⅱ:the lower curve areas of CPT and BNP are 0.902 and 0.826,respectively;cardiac function level Ⅲ:the lower curve areas of CPT and BNP are 0.653 and 0.690 respectively;cardiac function level Ⅳ:the lower levels of CPT and BNP The curve areas are 0.701 and 0.685,respectively.Correlation analysis showed that cardiac function classification was positively correlated with serum CPT and BNP levels in patients with CHF(P<0.05).Conclusion The cardiac function grade of CHF patients is positively correlated with the levels of serum CPT and BNP,which should be paid attention to in clinic.
