右美托咪定在无痛胃镜中的麻醉疗效评价及安全性刍议

Efficacy Evaluation and Safety of Dexmedetomidine in Painless Gastroscopy

目的探究右美托咪定在无痛胃镜中的麻醉效果和安全性。方法回顾性分析2020年10月—2021年10月期间于该院接受无痛胃镜检查的100例患者的一般资料,按照不同的麻醉方法,分为对照组和研究组,各50例。对照组采取丙泊酚静脉推注麻醉,研究组联用丙泊酚和右美托咪定。对比两组镇痛镇静效果、麻醉起效时间、术后苏醒时间、检查时间、定向力恢复时间、不良反应发生率。结果研究组镇痛镇静优良率(96.00%)高于对照组(74.00%),差异有统计学意义(χ^(2)=9.490,P<0.05);研究组麻醉起效时间、术后苏醒时间、检查时间、定向力恢复时间短于对照组,差异有统计学意义(P<0.05);检查时,研究组心率、舒张压、收缩压高于对照组,差异有统计学意义(P<0.05);研究组不良反应发生率(10.00%)与对照组(20.00%)相近,差异无统计学意义(χ^(2)=1.961,P>0.05)。结论右美托咪定不仅可以有效镇痛镇静,保证检查的顺利进行,还见效迅速、检查后使患者尽快苏醒,并且检查中不会使患者生命体征出现较大波动,其在无痛胃镜检查中有非常显著麻醉效果和安全性。

Objective To explore the anesthesia effect and safety of dexmedetomidine in painless gastroscopy.Methods A retrospective analysis on 100 patients general information who underwent painless gastroscopy in the hospital from October 2020 to October 2021.According to different anesthesia methods,they were divided into control group and study group,with 50 cases in each group.The control group was anesthetized by intravenous injection of propofol,and the study group was combined with propofol and dexmedetomidine.The analgesic and sedative effects,anesthe⁃sia onset time,postoperative recovery time,examination time,orientation recovery time,and incidence of adverse re⁃actions of two groups were compared.Results The excellent rate of analgesia and sedation in the study group(96.00%)was higher than that in the control group(74.00%),the difference was statistically significant(χ^(2)=9.490,P<0.05).The onset time of anesthesia,postoperative recovery time,examination time,and orientation recovery time in the study group were shorter than those in the control group,the difference was statistically significant(P<0.05).Dur⁃ing the examination,the heart rate,diastolic blood pressure and systolic blood pressure of the study group were higher than those of the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group(10.00%)was similar to that in the control group(20.00%),the difference was not statisti⁃cally significant(χ^(2)=1.961,P>0.05).Conclusion Dexmedetomidine can not only effectively relieve pain and seda⁃tion,and ensure the smooth progress of the examination,but also can take effect quickly,make the patient wake up as soon as possible after the examination,and will not make the patient's vital signs fluctuate greatly during the ex⁃amination.There are very significant anesthesia effects and safety in gastroscopy.
