米非司酮联合缩宫素在产后出血治疗中的应用及可行性评价

Application and Feasibility Evaluation of Mifepristone Combined with Oxytocin in the Treatment of Postpartum Hemorrhage

目的分析应用米非司酮联合缩宫素治疗产后出血的临床效果。方法随机选取该院2019年8月—2020年8月就诊的80例产后出血患者,依据抽样法进行等比分为对照组(n=40)和观察组(n=40),对照组采取单一缩宫素治疗与观察组采取米非司酮+缩宫素治疗,比较两组治疗效果。结果观察组总有效率97.50%高于对照组80.00%,差异有统计学意义(χ^(2)=4.507,P<0.05)。观察组血红蛋白含量(9.78±3.76)g/dl高于对照组,而转经经期(3.94±1.03)d、阴道出血量(46.38±6.22)mL、出血时间(3.97±2.78)d和转经时间(20.04±3.27)d均低于对照组,差异有统计学意义(t=2.463、6.934、8.136、5.362、10.953,P<0.05)。观察组PT(10.58±1.04)s、APTT(25.88±2.35)s、TT(16.78±1.76)s和FBI(3.95±0.39)g/L指标均优于对照组,差异有统计学意义(t=16.606、7.311、9.771、15.940,P<0.05)。观察组不良反应发生率2.50%低于对照组20.00%,差异有统计学意义(χ^(2)=4.507,P<0.05)。结论应用米非司酮+缩宫素治疗产后出血药效发挥强,加快恢复凝血功能,稳定血红蛋白含量,减少不良反应发生,临床用药安全性较高。

Objective To analyze the clinical effect of mifepristone combined with oxytocin in the treatment of post⁃partum hemorrhage.Methods A total of 80 patients with postpartum hemorrhage from August 2019 to August 2020 in the hospital were randomly selected and grouped in equal proportions according to the sampling method,it is di⁃vided into control group(n=40)and observation group(n=40).The control group were treated with single oxytocin,and the observation group were treated with mifepristone+oxytocin,and the therapeutic effects of the two groups were compared.Results The total effective rate of the observation group 97.50%was much higher than that of the control group 80.00%,the difference was statistically significant(χ^(2)=4.507,P<0.05).The hemoglobin content of the obser⁃vation group was(9.78±3.76)g/dl higher than that of the control group,while menstrual transition period(3.94±1.03)d,vaginal bleeding volume(46.38±6.22)mL,bleeding time(3.97±2.78)d and menstrual transition time(20.04±3.27)d were shorter than those in the control group,the difference was statistically significant(t=2.463,6.934,8.136,5.362,10.953,P<0.05).The PT(10.58±1.04)s,APTT(25.88±2.35)s,TT(16.78±1.76)s and FBI(3.95±0.39)g/L in⁃dexes of the observation group were better than those of the control group,the difference was statistically significant(t=16.606,7.311,9.771,15.940,P<0.05).The incidence of adverse reactions in the observation group 2.50%was lower than that in the control group 20.00%,the difference was statistically significant(χ^(2)=4.507,P<0.05).Conclu⁃sion The application of mifepristone+oxytocin in the treatment of postpartum hemorrhage exerts strong efficacy,ac⁃celerates the recovery of coagulation function,stabilizes the hemoglobin content,reduces the occurrence of adverse reactions,and has high clinical safety.
