血清CRP联合PCT检测对老年呼吸道细菌感染的诊断价值

Diagnostic Value of Serum CRP Combined with PCT Detection in Elderly Respiratory Bacterial Infection

目的探讨血清C反应蛋白(CRP)联合降钙素原(PCT)检测在老年呼吸道细菌感染患者中的诊断价值。方法选取2020年9月至2021年10月范县人民医院收治的共计90例老年呼吸道感染患者作为研究对象,按照病原菌菌种鉴定结果分成细菌感染组(57例)与非细菌感染组(33例),细菌感染组中革兰氏阳性(G^(+))菌感染31例,革兰氏阴性(G^(-))菌感染26例,比较各组CRP、PCT水平,分析CRP检测、PCT检测及联合检测的诊断结果、诊断价值(灵敏度、特异度、阳性预测值、阴性预测值、诊断准确率)。结果细菌感染组CRP、PCT水平高于非细菌感染组,差异有统计学意义(P<0.05);G^(+)菌感染组CRP、PCT水平低于G^(-)菌感染组,差异有统计学意义(P<0.05);联合检测阳性率高于CRP检测、PCT检测,差异有统计学意义(P<0.05);联合检测灵敏度、阴性预测值、诊断准确率高于CRP检测、PCT检测,差异有统计学意义(P<0.05)。结论老年呼吸道细菌感染患者的血清CRP、PCT水平变化明显,给予CRP联合PCT检测,能够辅助鉴别病原菌种类,提高诊断结果准确性,且灵敏度、阴性预测值、诊断准确率均较高。

Objective To investigate the diagnostic value of serum C-reactive protein(CRP)combined with procalcitonin(PCT)detection in elderly patients with respiratory bacterial infection.Methods A total of 90 elderly patients with respiratory tract infection who were admitted in Fanxian People's Hospital from September 2020 to October 2021 were selected as the research objects.According to the identification results of pathogenic bacteria,they were divided into the bacterial infection group(57 cases)and the non-bacterial infection group(33 cases),31 cases of gram-positive(G^(+))bacteria infection and 26 cases of gram-negative(G^(-))bacteria infection in the bacterial infection group.The levels of CRP and PCT in each group were compared,and the diagnostic results and diagnostic value(sensitivity,specificity,positive predictive value,negative predictive value,diagnostic accuracy)of CRP detection,PCT detection and combined detection were analyzed.Results Compared with the non-bacterial infection group,the levels of CRP and PCT in the bacterial infection group were higher(P<0.05).Compared with the G^(-)bacteria infection group,the levels of CRP and PCT in the G^(+)bacteria infection group were lower(P<0.05).The positive rate of combined detection was higher than that of CRP detection and PCT detection(P<0.05).Compared with CRP detection and PCT detection,the sensitivity,negative predictive value and diagnostic accuracy of the combined detection were higher(P<0.05).Conclusion The serum CRP and PCT levels of elderly patients with respiratory bacterial infection change significantly.The combination of CRP and PCT can help identify pathogenic bacteria and improve the accuracy of the diagnostic results,the sensitivity,negative predictive value and diagnostic accuracy are all high.
