摘要
目的 探讨社区老年人群谷氨酰转肽酶(GGT)与MS的关系,评价GGT对MS的影响。方法 选取2015年4~9月于北京市石景山区古城及苹果园社区居住的符合入选标准≥60岁老年人4744名,进行标准问卷调查和生化指标检测,根据GGT四分位法分为Q1(GGT<15.6 U/L)组1180名,Q2(15.6≤GGT<20.3 U/L)组1177名,Q3(20.3≤GGT<28.2 U/L)组1195名,Q4(GGT≥28.2 U/L)组1192名,分析比较各组指标。结果 MS组GGT水平高于非MS组[男性27.5(21.0,38.4)vs 21.7(16.7,30.4)U/L,女性21.9(17.0,29.4)vs 17.3(13.9,23.1)U/L,P<0.01]。Q1、Q2、Q3和Q4组MS患病率逐渐升高(14.0%vs 21.9%vs 34.0%vs 41.3%,P<0.05)。Logistics回归分析显示,BMI、血尿酸、GGT是MS的影响因素,Q3、Q4组MS风险分别增加1.543倍和1.489倍。随访4年后Q4组新发MS患病率高于Q1、Q2组(40.9%vs 22.0%vs 24.8%,P<0.05)。结论 社区老年人群GGT水平较高,随着GGT升高,MS患病率升高,GGT是MS的影响因素。
ObjectiveTo investigate the relationship between glutamyl transpeptidase(GGT)and metabolic syndrome(MS)in elderly populationin community,and to evaluate the effect of GGT on MS.MethodsA total of 4744 subjects over 60 years old who lived in Gucheng and Pingguoyuan communities in Shijingshan District of Beijing were enrolled in this study from April to September 2015.All the participants underwent standard questionnaire survey and biochemical tests.According to GGT level,they were divided into 4 groups:Q1 group(GGT< 15.6 U/L,n=1180),Q2 group(15.6≤GGT<20.3 U/L,n=1177),Q3 group(20.3 ≤ GGT <28.2 U/L,n=1195),and Q4 group(GGT ≥ 28.2 U/L,n=1192).Clinical characteristics were compared among the groups.group than in non MS group[27.5(21.0,38.4)vs 21.7(16.7,30.4)U/L in male population and 21.9(17.0,29.4)vs 17.3(13.9,23.1)U/L,P<0.01 in female population].There were significant differences in the prevalence of MS among Q1,Q2,Q3 and Q4 group(14.0% vs 21.9% vs 34.0% vs 41.3%,P<0.05).Logistic regression model analysis showed that BMI,uric acid and GGT were independent influencing factors for MS.When GGT was at the third and fourth levels of high level,the risk of MS was 1.543 times and1.489 times than first level of low level respectively.After 4 years of follow-up,the incidence of new MS was higher in Q4 group than in Q1 and Q2 group(40.9% vs 22.0% vs 24.8%,P<0.05).The level of GGT is increasedin elderly patients with MS in community.With the increase of GGT,the prevalence of MS gradually increases.GGT is an independent influencing factor for MS.
