摘要
目的 探讨新型冠状病毒肺炎(COVID-19)对住院成人 T1DM 患者血糖控制的影响。方法 选取 2019 年1月至 2021 年 1 月于北京大学第三医院内分泌科住院的成人 T1DM 患者 106 例。以疫情发生前、后 1 年为时间界限,分为疫情前组(n=64)和疫情后组(n=42),分析比较两组年龄、性别、DM 病程、身高、体重、BP、FPG、HbA;c、血脂、肝功能等,以及 DKA 发生率及住院期间 Ins 用量差异。 结果 疫情后组 HbA;c 水平、DKA 发生率高于疫情前组[(11.2±2.86)% vs(10.1±2.62)%,24% vs 6%,P<0.05]。疫情前后每日 Ins 用量差异无统计学意义(P>0.05)。 结论 在 COVID-19疫情流行期间,住院成人 T1DM 患者血糖控制较差,DKA 发生风险较高。
Objective To investigate the effect of novel coronavirus(COVID - 19)pandemic on glycemic control in hospitalized adults with type 1 diabetes mellitus(T1DM).Methods 106 adult inpatients with T1DM were included from the Department of Endocrinology of Peking University Third Hospital from January 2019 to January 2021.They were divided into two groups according to the outbreak before(n=64)and after(n=42)COVID - 19 pandemic.Medical history and physical examination data,including age,sex,the duration of diabetes,body height,weight,and blood pressure,were collected.Fasting plasma glucose,glycated hemoglobin A1c,blood lipid profile and liver function were also evaluated.The incidence of diabetic ketoacidosis(DKA)and daily insulin dose during hospitalization were recorded.Results Compared with the T1DM group hospitalized before the pandemic,the levels of HbA;c[(11.2±2.86)% vs(10.1±2.62)%,P<0.05]and the incidence of DKA[24% vs 6%,P<0.05]were higher in the group going through the pandemic.There was no significant difference in total daily insulin dose between the two groups.Conclusion Poorer glycemic control and higher DKA risk can be found in adult hospitalized patients with T1DM during COVID-19 pandemic.
