哮喘患者居家丙酸氟替卡松吸入气雾剂的用药指导

Medication guidance of Fluticasone Propionate Inhaled Aerosol in asthmatic patients at home

目的构建哮喘患者居家丙酸氟替卡松吸入气雾剂用药指导模式,并研究其效果。方法研究纳入老年哮喘患者102例,将患者分成2组,各51例。对照组患者进行常规治疗;观察组患者(51例)进行常规治疗后,由药师指导患者居家进行丙酸氟替卡松吸入气雾剂治疗,比较2组患者疗效、症状及体征(气促、喘息、咳嗽和肺部哮鸣音等)消失时间,治疗前、后2组患者日间与夜间症状评分变化,功能指标及血清炎症因子指标变化,治疗依从性良好率,治疗后生活质量提升率和不良反应发生的情况。结果观察组有效率(94.12%)高于对照组(80.39%),P<0.05;2组患者治疗前日间与夜间症状评分、肺功能指标及血清炎症因子指标相近,治疗后各组患者日间及夜间症状评分、肺功能指标及血清炎症因子指标均改善,观察组患者治疗后日间及夜间症状评分、肺功能指标及血清炎症因子指标优于对照组(P<0.05);观察组患者症状与体征消失时间均短于对照组,且观察组患者治疗依从性良好率(94.12%)高于对照组(78.43%),生活质量提升率(88.24%)高于对照组(62.75%),P<0.05;观察组出现1例失眠,对照组出现1例乏力、1例腹泻,2组不良反应率分别为1.96%、3.92%,均较低,P>0.05。结论药学服务在哮喘患者居家丙酸氟替卡松吸入气雾剂治疗中作用显著,患者疗效提升,症状改善,肺功能恢复较好,且患者依从性好,生活质量提升,安全可行。

Objective To study the construction and effect of medication guidance model of fluticasone propionate inhaled aerosol in asthmatic patients at home.Methods 102 elderly asthmatic patients in the community were included in this study,and the patients were divided into 2 groups with 51 patients in each group.Patients in the control group received routine treatment and inhaled Fluticasone Propionate Aerosol at home.Patients in the observation group were instructed by pharmacists after routine treatment to inhale the aerosol of fluticasone propionate.The efficacy,symptoms and signs(shortness of breath,wheezing,cough,lung wheezing,etc.)disappeared time,daytime and night symptom scores,pulmonary function indexes and serum inflammatory factors,treatment compliance rate,quality of life improvement rate and adverse reactions were compared between the 2 groups before and after treatment.Results The effective rate of the observation group(94.12%)was higher than that of control group(80.39%),P<0.05.After treatment,the daytime and night time symptom scores,pulmonary function index and serum inflammatory factor index were improved,and the observation group is better than control group(P<0.05).The disappearance time of symptoms and signs in observation group was shorter than that in control group.The treatment compliance rate of observation group(94.12%)was higher than that in control group(78.43%),and the improvement rate of quality of life(88.24%)was higher than that in control group(62.75%),P<0.05.There was 1 case of insomnia in the observation group,1 case of fatigue and 1 case of diarrhea in the control group.The adverse reaction rates in the 2 groups were 1.96%and 3.92%,respectively,indicating no significant difference between the groups(P>0.05).Conclusion The pharmaceutical interventions implemented on asthma patients at home using Fluticasone Propionate Aerosol improved the curative effect and had good patient compliance.
