血清细胞间黏附分子-1对重症肺炎患儿临床治疗效果的诊断与治疗评估价值

摘要 目的 探讨血清细胞间黏附分子-1对重症肺炎患儿临床治疗效果的诊断与治疗评估价值。方法 选择2019年2月—2021年9月嘉祥县人民医院收治的80例重症肺炎患儿为研究对象,按照是否检测sICAM-1进行临床治疗指导分为两组,各40例。两组均行抗菌治疗,观察组定期检测患儿sICAM-1水平;对照组行常规治疗,比较两组使用抗菌药物治疗时间、住院时间及并发症。结果 观察组使用抗菌药物治疗时间及住院时间分别为(6.1±0.1)、(7.2±0.2)d,均显著短于对照组的(9.2±0.3)、(10.3±0.4)d,差异有统计学意义(t=62.000、43.841,P<0.05),观察组发生呼吸衰竭、肝肾功能障碍、心力衰竭及肠麻痹等常见重症肺炎相关并发症为4例(10.0%)显著低于对照组的20例(50.0%),差异有统计学意义(χ^(2)=15.238,P<0.05)。结论 针对重症肺炎患儿使用ICAM水平检测进行诊断与预后判断,能有效缩短患儿住院时间,提高抗菌药物治疗效率,减少并发症,从而提高临床治疗效果。 Objective To investigate the value of serum intercellular adhesion molecule-1 in the diagnosis and treatment evaluation of the clinical treatment effect of children with severe pneumonia.MethodsA total of 80 children with severe pneumonia admitted to Jiaxiang County People’s Hospital from February 2019 to September 2021 were selected as the research object.According to whether to detect sICAM-1 for clinical treatment guidance,they were divided into two groups,40 cases in each group.Antibacterial treatments were performed in the two groups.The observation group regularly detected the sICAM-1 level of the children.The control group was given routine treatment,and the duration of antibiotic treatment,hospital stay,and complications were compared between the two groups.ResultsThe duration of antibiotic treatment and hospital stay in the observation group were(6.1±0.1) d,(7.2±0.2) d,respectively,both were significantly shorter than the control group(9.2±0.3) d,and(10.3±0.4) d,the difference was statistically significant(t=62.000,43.841,P<0.05).The total proportion of common severe pneumonia-related complications such as respiratory failure,liver and kidney dysfunction,heart failure and intestinal paralysis in the observation group was 4 cases(10.0%),which was significantly lower than 20 cases(50.0%) in the control group,the difference was statistically significant(χ^(2)=15.238,P<0.05).ConclusionThe use of ICAM level detection for diagnosis and prognosis judgment of patients with severe pneumonia can effectively shorten the hospitalization time of patients,improve the efficiency of antibiotic treatment,reduce complications,and improve the clinical treatment effect.
