阴道彩色多普勒超声参数与卵巢癌患者疾病分期和MVD、Ang-2表达的关联性

Correlation of vaginal color Doppler ultrasound parameters with disease stage and expression of MVD and Ang-2 in patients with ovarian cancer

目的分析阴道彩色多普勒超声参数与卵巢癌患者疾病分期和微血管密度(MVD)、促血管生成素-2(Ang-2)表达的关联性。方法抽取2020年3月至2021年5月鹤壁市人民医院收治的卵巢癌患者90例为卵巢癌组,并以1∶1配比抽取同期收治的卵巢良性肿瘤患者90例为良性组,均进行阴道彩色多普勒超声和MVD、Ang-2检测,统计两组和卵巢癌组不同分期者阴道彩色多普勒超声参数[阻力指数(RI)、搏动指数(PI)、最高峰流速(Vmax)、平均流速(VM)、舒张末期流速(EDV)]及MVD、Ang-2表达情况,分析超声参数与疾病分期和MVD、Ang-2的相关性。结果卵巢癌组PI、RI低于良性组,Vmax、VM、EDV高于良性组(P<0.05)。卵巢癌组MVD、Ang-2阳性率高于良性组(P<0.05)。卵巢癌组Ⅳ期者PI和RI低于Ⅰ、Ⅱ、Ⅲ期者,Vmax、VM及EDV高于Ⅰ~Ⅲ期者(P<0.05)。卵巢癌组Ⅳ期者MVD表达和Ang-2阳性率高于Ⅰ、Ⅱ、Ⅲ期者(P<0.05)。Pearson分析显示,PI(r=-0.54、-0.59、-0.61,P均<0.05)和RI(r=-0.61、-0.64、-0.66,P均<0.05)与疾病分期、MVD、Ang-2呈负相关,V_(max)(r=0.65、0.66、0.67,P均<0.05)、VM(r=0.69、0.71、0.70,P均<0.05)、EDV(r=0.68、0.69、0.68,P均<0.05)与其呈正相关。结论卵巢癌患者阴道彩色多普勒超声参数在评估疾病分期和肿瘤新生血管情况中具有良好应用价值,可对肿瘤性质和疾病进展进行评估鉴定。

Objective To analyze the correlation of vaginal color Doppler ultrasound parameters with disease stage,expression of microvascular density(MVD)and angiopoietin-2(Ang-2)in patients with ovarian cancer.Methods A total of 90 ovarian cancer patients admitted to Hebi People’s Hospital from March 2020 to May 2021 were selected as the ovarian cancer group,and 90 patients with benign ovarian tumors during the same period were selected as the benign group with a ratio of 1∶1 with the ovarian cancer group.Vaginal color Doppler ultrasonography,MVD and Ang-2 detection were performed on all patients.The vaginal color Doppler ultrasound parameters such as resistance index(RI),pulsatility index(PI),maximum velocity(Vmax),mean flow velocity(VM),end-diastolic flow velocity(EDV),expression of MVD and Ang-2 in the two groups and in patients with different stages of ovarian cancer group were counted.The correlation of ultrasound parameters with disease stage,MVD and Ang-2 was analyzed.Results The PI and RI in ovarian cancer group were lower than those in benign group,while the Vmax,VM and EDV were higher than those in benign group(P<0.05).The expression of MVD and positive rate of Ang-2 in ovarian cancer group were higher than those in benign group(P<0.05).In ovarian cancer group,the PI and RI of patients at stage IV were lower than those of patients at stageⅠ,ⅡandⅢ,while the Vmax,VM and EDV were higher than those of patients at stageⅠ-Ⅲ(P<0.05).The expression of MVD and the positive rate of Ang-2 in patients at stageⅣwere higher than those in patients at stageⅠ,ⅡandⅢ(P<0.05).Pearson analysis showed that PI(r=-0.54,-0.59,-0.61,all P<0.05)and RI(r=-0.61,-0.64,-0.66,all P<0.05)were negatively correlated with disease stage,MVD,and Ang-2;while V_(max)(r=0.65,0.66,0.67,all P<0.05),VM(r=0.69,0.71,0.70,all P<0.05),and EDV(r=0.68,0.69,0.68,all P<0.05)were positively correlated with disease stage,MVD,and Ang-2.Conclusions Transvaginal color Doppler ultrasound parameters in ovarian cancer patients have good application value in evaluating disease staging and tumor neovascularization,and can evaluate and identify tumor properties and disease progression.
