中晚期大肠癌中医证型与凝血指标的相关性

Correlation Between TCM Syndrome Type and Coagulation Index of Middle and Advanced Colorectal Cancer

目的:探讨凝血指标与中晚期大肠癌中医证型的相关性。方法:分析100例中晚期大肠癌患者中医证型的分布、临床凝血指标与中晚期大肠癌中医证型之间的关系。结果:气滞血瘀证与凝血酶时间(thrombin time,TT)呈负相关(OR=0.369,P=0.001<0.05);与纤维蛋白原水平(fibrinogen,FIB)呈正相关(OR=13.541,P=0.000<0.05);与D-二聚体(D-dimer,D-D)呈正相关(OR=1.182,P=0.045<0.05)。肝肾阴虚证与FIB呈负相关(OR=0.121,P=0.003<0.05)。湿热蕴结证与TT呈正相关(OR=2.119,P=0.015<0.05),与FIB呈负相关(OR=0.144,P=0.002<0.05)。结论:中晚期大肠癌患者TT时间缩短、FIB和D-D升高时更易发生气滞血瘀证;FIB水平较低时,则更易出现肝肾阴虚证;TT时间延长、FIB降低时,多为湿热蕴结证。

Objective:To explore the correlation between coagulation indexes and TCM syndromes of middle and advanced colorectal cancer.Methods:The distribution of TCM syndromes,and the relationship between clinical coagulation indexes and TCM syndromes of middle and advanced colorectal cancer of 100 patients with middle and advanced colorectal cancer were analyzed.Results:There was a negative correlation between QI stagnation Blood stasis syndrome and TT(OR=0.369,P=0.001<0.05).It was positively correlated with FIB(OR=13.541,P=0.000<0.05).It was positively correlated with D-D(OR=1.182,P=0.045<0.05).Liver and Kidney Yin deficiency syndrome was negatively correlated with FIB(OR=0.121,P=0.003<0.05).Dampness and Heat accumulation syndrome was positively correlated with TT(OR=2.119,P=0.015<0.05).It was negatively correlated with FIB(OR=0.144,P=0.002<0.05).Conclusion:In patients with middle and advanced colorectal cancer,the TT time is shortened,and Qi stagnation and Blood stasis syndrome is more likely to occur when FIB and D-D are elevated.When FIB level is low,Liver and Kidney Yin deficiency syndrome is more likely to occur.When TT time is prolonged and FIB level is decreased,Dampness and Heat accumulation syndrome should be considered.
