基于医联体健康模式下的血糖控制对老年糖尿病患者的干预效果

Intervention Effect of Blood Glucose Control on Elderly Diabetic Patients Based on Medical Alliance Health Model

目的分析老年糖尿病患者应用以医联体健康模式下开展血糖控制干预的效果。方法选择2021年1—12月收治的150例老年糖尿病患者,根据随机数表法分为对照组和研究组,各75例。对照组患者应用常规血糖控制干预,研究组患者基于医联体健康模式下的血糖控制干预。观察两组患者的干预效果、血糖指标、尿微量白蛋白水平(尿MA)及不良反应发生率。结果干预后,研究组患者的糖化血红蛋白水平(HbA1c)、尿MA、空腹血糖水平、餐后2 h血糖水平分别为(6.3±1.2)%、(7.3±0.8)μg/mL、(6.87±0.51)mmol/L、(8.81±0.63)mmol/L,优于对照组,差异有统计学意义(t=5.636、59.617、10.596、9.431,P<0.05)。研究组总有效率、不良反应发生率分别为96.00%、8.00%,优于对照组,差异有统计学意义(χ^(2)=4.127、8.112,P<0.05)。结论将基于医联体健康模式下的血糖控制干预应用于老年糖尿病患者中,可以改善患者的各项血糖相关指标的水平,同时提升患者的干预效果和降低患者的不良反应发生率。

Objective To analyse the intervention effect of carry out blood glucose control intervention in elderly patients with diabetes mellitus under the health model of medical union.Methods A total of 150 elderly diabetic patients admitted from January to December 2021 were selected.According to the random number table method,they were divided into the control group and the study group with 75 cases each.The patients in the control group were given routine blood glucose control intervention,while the patients in the study group were given blood glucose control intervention based on the medical alliance health model.The intervention effects,blood glucose index,the levels of urine microalbumin(MA)and the incidence of adverse reactions of the two groups of patients were observed.Results After intervention,the levels of glycosylated hemoglobin(HbA1c)urine MA,fasting blood glucose,and 2 h postprandial blood glucose levels in the study group were(6.3±1.2)%,(7.3±0.8)μg/mL,and(6.87±0.51)mmol/L,and(8.81±0.63)mmol/L,which were better than those of the control group,and the difference was statistically significant(t=5.636,59.617,10.596,9.431,P<0.05).The total effective rate and the incidence of adverse reactions in the study group were 96.00%and 8.00%,respectively,which were better than those in the control group,and the difference was statistically significant(χ^(2)=4.127,8.112,P<0.05).Conclusion Applying the blood glucose control intervention based on the medical alliance health model to elderly diabetic patients can improve the level of various blood glucose related indicators in patients,and at the same time improve the intervention effect of patients and reduce the incidence of adverse reactions in patients.
