杜仲颗粒联合地屈孕酮片治疗早期先兆流产肾虚证临床研究

Clinical Study on Duzhong Granules Combined with Dydrogesterone Tablets for Early Threatened Abortion with Kidney Deficiency Syndrome

目的:观察杜仲颗粒联合地屈孕酮片治疗早期先兆流产肾虚证的疗效。方法:选取92例早期先兆流产肾虚证患者随机分为治疗组和对照组各46例。2组均服用地屈孕酮片,治疗组加服杜仲颗粒治疗。2组疗程均为2周。治疗前后评定肾虚证症状评分、检测血清性激素[人绒毛膜促性腺激素(β-HCG)、孕酮(P)、雌二醇(E2)]水平。比较2组的临床疗效。随访12个月,比较2组的足月妊娠率、早产率及流产率。结果:治疗2周,治疗组的临床疗效总有效率高于对照组(P<0.05)。2组阴道出血量、阴道出血色质、腰膝酸软、小腹坠痛、夜尿频多、头晕耳鸣评分均较治疗前降低(P<0.01),治疗组6项评分均低于对照组(P<0.01)。2组血清β-HCG、P、E2水平均较治疗前升高(P<0.01),治疗组血清β-HCG、P、E2水平均高于对照组(P<0.01)。随访12个月,治疗组的足月妊娠率高于对照组,早产率低于对照组,差异均有统计学意义(P<0.01,P<0.05);治疗组的流产率稍低于对照组,差异无统计学意义(P>0.05)。结论:杜仲颗粒联合地屈孕酮片治疗早期先兆流产肾虚证的疗效显著,可有效缓解症状、调节性激素水平,提高保胎效果。

Objective:To observe the effect of the therapy of Duzhong granules combined with dydrogesterone tablets on the treatment of early threatened abortion with kidney deficiency syndrome.Methods:A total of 92 patients with early threatened abortion with kidney deficiency syndrome were randomly divided into the treatment group and the control group,with 46 cases in each group.Both groups were treated with dydrogesterone tablets,and the treatment group was treated with Duzhong granules.The treatment course of both groups was two weeks.Before and after treatment,the symptom scores of kidney deficiency syndrome were evaluated,and the levels of human chorionic gonadotropin(β-HCG),progesterone(P)and estradiol(E2)in serum were detected.The clinical effects were compared between the two groups.The full-term pregnancy rate,premature birth rate and abortion rate in the two groups were compared during the follow-up of12 months.Results:After two weeks of treatment,the total effective rate in the treatment group was higher than that in the control group(P<0.05).Compared with those before treatment,the scores of vaginal bleeding amount,color and quality of vaginal bleeding,soreness and weakness in waist and knee,falling pain in lower abdomen,frequent night urination,dizziness and tinnitus in both groups were decreased(P<0.01),and the above six scores in the treatment group were lower than those in the control group(P<0.01).The levels ofβ-HCG,P and E2in serum in both groups were increased when compared with those before treatment(P<0.01),and the above three levels in the treatment group were higher than those in the control group(P<0.01).After 12 months of follow-up,the full-term pregnancy rate in the treatment group was higher than that in the control group,and the premature birth rate was lower,differences being significant(P<0.01,P<0.05).The abortion rate in the treatment group was slightly lower than that in the control group,there being no significant difference(P>0.05).Conclusion:The therapy of Duzhong granules combined with dydrogesterone tablets has a significant effect on the treatment of early threatened abortion with kidney deficiency syndrome,which can effectively relieve symptoms,regulate the levels of sex hormones and improve the effect of fetal protection.
