摘要 目的:分析2型糖尿病患者血糖水平与睡眠障碍的相关性。方法:选取2019年9月至2020年10月东莞仁康医院收治的2型糖尿病患者70例作为研究对象,按照匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)评分结果分为观察组(n=50)和对照组(n=20)。其中观察组PSQI评分>5分,对照组PSQI评分≤5。检测并比较2组患者血糖多指标水平差异。对2组患者进行身高、体质量测量,后计算体质量指数值(BMI)。测量2组患者空腹血糖(FPG)水平、餐后2 h血糖(2 h PG)水平及糖化血红蛋白(HbA1c)水平。根据HbA1c检测水平将所有患者分为血糖控制达标组(HbA1c<7%,n=53)与血糖控制未达标组(≥7%,n=17),另评估2组患者PSQI评分。结果:2型糖尿病患者睡眠障碍发生率为71.43%;观察组血浆皮质醇、HbA1c水平明显高于对照组;血糖控制达标组睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、催眠药物及日间功能障碍项目评分均低于血糖控制未达标组,2组比较差异均有统计学意义(均P<0.05);2型糖尿病患者血糖水平与其睡眠障碍发生风险正相关(P<0.05)。结论:血浆皮质醇、HbA1c水平异常升高均可能导致2型糖尿病患者睡眠障碍发生,临床针对患者上述指标变化进行密切监测可有效提高患者睡眠质量,改善其预后结局。 Objective:To analyze the correlation between blood glucose level and sleep disorders in patients with diabetes mellitus type 2.Methods:A total of 70 patients with diabetes mellitus type 2 who were admitted to Dongguan Renkang Hospital from September 2019 to October 2020 were selected as the research subjects.According to the Pittsburgh Sleep Quality Index(PSQI)score,they were divided into observation group and control group.There were 50 cases in the observation group and 20 cases in the control group.The PSQI score of the observation group was>5,and the PSQI score of the control group was≤5.The blood glucose levels of the two groups were detected and compared.The height and weight of patients in the two groups were measured,and body mass index(BMI)was calculated.Fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG)and glycosylated hemoglobin(HbA1c)levels were measured between 2 groups.According to the HbA1c level,all patients were divided into the blood glucose control standard group(HbA1c<7%,n=53)and the blood glucose control standard group(≥7%,n=17).The PSQI score of patients in the two groups was also evaluated.Results:The incidence of sleep disorders in diabetes mellitus type 2 patients was 71.43%.The levels of plasma cortisol and HbA1c in observation group were significantly higher than those of the control group.The scores of sleep quality,sleep latency,sleep time,sleep efficiency,sleep disorder,hypnotic drugs and daytime dysfunction in the glucose control group were lower than those in the glucose control group,and the differences were statistically significant(Ps<0.05).There was a positive correlation between blood glucose level and the risk of sleep disturbance in diabetes mellitus type 2 patients(P<0.05).Conclusion:Abnormal increase of plasma cortisol and HbA1c levels may lead to sleep disorders in patients with diabetes mellitus type 2.Clinical monitoring of these changes can effectively improve sleep quality and prognosis of patients.
