血清抗米勒管激素联合黄体生成素与卵泡刺激素比值对多囊卵巢综合征的诊断价值

Diagnostic value of serum anti-Mullerian hormone combined with luteinizing hormone and follicle stimulating hormone ratio in polycystic ovary syndrome

目的探讨血清抗米勒管激素(AMH)联合黄体生成素(LH)与卵泡刺激素(FSH)比值(LH/FSH)对多囊卵巢综合征(PCOS)的诊断价值。方法选取2020年1月至2021年5月在广西壮族自治区生殖医院确诊的112例PCOS患者作为PCOS组,并选取同期入院体检的非PCOS健康女性87例作为对照组。用罗氏Cobas e601全自动电化学发光分析仪检测两组的血清AMH、LH、FSH水平,计算LH/FSH比值,比较两组检测指标水平差异,采用受试者工作特征(ROC)曲线探究血清AMH水平联合LH/FSH比值对PCOS的诊断价值。结果PCOS组的AMH、LH水平及LH/FSH比值高于对照组,FSH水平低于对照组,差异均有统计学意义(P<0.05)。ROC曲线分析显示,血清AMH水平联合LH/FSH比值诊断PCOS的曲线下面积(AUC=0.901,95%CI:0.858~0.943)最大,敏感度和特异性分别为83.0%和81.6%。结论PCOS患者血清AMH水平和LH/FSH比值高于非PCOS健康女性,两者联合检测对PCOS有较高的诊断价值,可作为有效的参考指标,值得临床推广应用。

Objective To investigate the diagnostic value of serum anti-Müllerian hormone(AMH)combined with luteinizing hormone(LH)and follicle stimulating hormone(FSH)ratio(LH/FSH)in polycystic ovary syndrome(PCOS).Methods A total of 112 patients with PCOS diagnosed in the Reproductive Hospital of Guangxi Zhuang Autonomous Region from January 2020 to May 2021 were selected as the PCOS group,and 87 non PCOS healthy women for physical examination in the same period were selected as the control group.Roche Cobas e601 automatic electrochemiluminescence analyzer was used to detect the levels of serum AMH,LH and FSH in the two groups,the ratio of LH/FSH was calculated,the difference of detection index levels was compared between the two groups,and the diagnostic value of serum AMH level combined with LH/FSH ratio for PCOS was explored by using receiver operating characteristic(ROC)curve.Results The levels of AMH,LH and LH/FSH in the PCOS group were higher than those in the control group,and the level of FSH was lower than that in the control group,the differences were statistically significant(P<0.05).ROC curve analysis showed that the area under the curve(AUC=0.901,95%CI:0.858-0.943)of serum AMH level combined with LH/FSH ratio in the diagnosis of PCOS was the largest,and its sensitivity and specificity were 83.0%and 81.6%respectively.Conclusion The serum AMH level and LH/FSH ratio in patients with PCOS are higher than those in non PCOS healthy women.The combined detection of the two has high diagnostic value for PCOS.It can be used as an effective reference index and is worthy of clinical application.
