围绝经期排卵障碍性异常子宫出血的中医体质特点

Traditional Chinese medicine constitution characteristics of abnormal uterine bleeding-ovulatory dysfunction in perimenopause

目的探究围绝经期排卵障碍性异常子宫出血的中医体质特点。方法对2018年3月至2021年3月华北石油管理局总医院妇产科和中医科门诊的359例围绝经期排卵障碍性异常子宫出血患者进行中医体质分析,探讨中医体质类型与围绝经期排卵障碍性异常子宫出血相关指标及血液指标的关系。结果围绝经期排卵障碍性异常子宫出血的中医体质分布以阴虚质、气虚质、湿热质、血瘀质为主。阴虚质评分与经期长度、子宫内膜厚度、促卵泡激素(FSH)及雌二醇(E2)呈正相关(P<0.05),气虚质评分与月经周期及血红蛋白(Hb)呈负相关(P<0.05),湿热质评分与经期长度呈正相关(P<0.05),血瘀质评分与子宫内膜厚度呈正相关(P<0.05),痰湿质评分与E2呈负相关(P<0.05)。结论阴虚质、气虚质、湿热质、血瘀质与围绝经期排卵障碍性异常子宫出血的发生密切相关。

Objective To explore the characteristics of traditional Chinese medicine constitution of abnormal uterine bleeding-ovulatory dysfunction in perimenopause.Methods From March 2018 to March 2021,the traditional Chinese medicine constitution of 359 patients with perimenopausal abnormal uterine bleeding-ovulatory dysfunction in the Outpatient Department of Obstetrics and Gynecology and Traditional Chinese Medicine of Huabei Petroleum Administration Bureau General Hospital was analyzed to explore the relationship between traditional Chinese medicine constitution types and related indexes and blood indexes of abnormal uterine bleeding-ovulatory dysfunction in perimenopause.Results The traditional Chinese medicine constitution distribution of perimenopausal abnormal uterine bleeding-ovulatory dysfunction was mainly yin deficiency,qi deficiency,damp heat and blood stasis.Yin deficiency score was positively correlated with menstrual period length,endometrial thickness,follicle-stimulating hormone(FSH)and estradiol(E2)(P<0.05).Qi deficiency score was negatively correlated with menstrual cycle and hemoglobin(Hb)(P<0.05).Damp heat score was positively correlated with menstrual period length(P<0.05).Blood stasis score was positively correlated with endometrial thickness(P<0.05),and phlegm dampness score was negatively correlated with E2(P<0.05).Conclusion Yin deficiency,qi deficiency,damp heat and blood stasis are closely related to the occurrence of abnormal uterine bleeding-ovulatory dysfunction i n perimenopause.
