集束化管理在青年女性自身免疫性脑炎患者中的应用效果

Application effect of cluster management in young female patients with autoimmune encephalitis

目的探讨集束化管理应用于青年女性自身免疫性脑炎(AE)患者的效果。方法选取2020年1月至2021年2月在安徽省立医院诊治的56例青年女性AE患者,通过随机数字表法分为常规组与集束化组,每组各28例。住院期给予常规组常规护理,给予集束化组集束化管理,干预时间均为4周。观察两组的护理质量(医院自拟调查量表)、患者认知功能[简易智力状态检查量表(MMSE)评分、8条目痴呆筛查问卷(AD8)评分]改善、意外事件(自伤、伤人)发生情况。结果干预后,集束化组的基础护理、专业护理及护理安全评分、总评分高于常规组,差异均有统计学意义(P<0.05);集束化组干预后的MMSE评分高于常规组,AD8评分低于常规组,差异均有统计学意义(P<0.05);集束化组的毁物、自伤、伤人等意外事件总发生率为14.29%(4/28),低于常规组的46.43%(13/28),差异有统计学意义(P<0.05)。结论青年女性AE患者住院治疗期间加强集束化护理管理,可明显提高总体护理质量,有助于患者认知功能改善,降低意外事件发生风险。

Objective To explore the effect of cluster management in young female patients with autoimmune encephalitis(AE).Methods Fifty six young female patients with AE who were treated in Anhui Provincial Hospital from January 2020 to February 2021 were selected and divided into the conventional group and the cluster group by random number table method,28 cases in each group.During hospitalization,the conventional group was given routine nursing care and the cluster group was given the cluster management.The intervention duration was 4 weeks.The nursing quality(hospital self-made questionnaire),the improvement of patients’cognitive function(mini-mental state examination[MMSE]score,8-item ascertain dementia[AD8]score),and the occurrence of accidents(selfinjury,injury,etc.)were observed between the two groups.Results After intervention,the scores of basic nursing,professional nursing,nursing safety and total score in the cluster group were higher than those in the conventional group(P<0.05).After intervention,the MMSE score in the cluster group was higher than that in the conventional group,and the AD8 s core was lower than that in the conventional group(P<0.05).The total incidence rate of accidental events such as destruction of objects,self-injury,and injury in the cluster group was 14.29%(4/28),which was lower than 46.43%(13/28)in the conventional group(P<0.05).Conclusion Strengthening cluster nursing management during hospitalization of young female AE patients,can significantly improve the overall care and treatment,help patients improve cognitive function,reduce the risk of acciden ts.
