华法林抗凝治疗的个体化药学监护

The individualized pharmaceutical care of anticoagulant treatmentwith Warfarin

目的:通过临床药师参与临床使用华法林患者的治疗,调整华法林国际标准化比值(INR)目标值,实施个体化药学监护和用药教育。方法:临床药师结合工作实践,总结参与抗凝治疗过程中遇到的问题和解决方案。结果:个体化的药学监护和用药教育使患者用药依从性明显提高,保证了INR在有效治疗范围,提高疗效的同时规避不良反应。结论:临床药师参与心血管疾病中抗凝治疗的个体化药学监护,可促进临床安全、有效、合理地使用华法林。

Objective:Based on the pharmaceutical treatment of patients who have been injected Warfarin,the individualized pharmaceutical care and drug use education will be implemented by adjusting the INR(International Normalized Ratio)target value of Warfarin.Methods:The problems encountered during the anticoagulant treatment process and solutions shall be summarized by combining the practice of clinical pharmacist.Results:The individualized pharmaceutical care and drug use education have significantly improved the drug use compliance of patients and guaranteed that the INR is within the effective treatment range.It has improved the curative effect while circumventing the adverse reaction.Conclusion:The participation of clinical pharmacist in the individualized pharmaceutical care of anticoagulant therapy in cardiovascular disease has promoted the safe,effective,and rational use of Warfarin.
