经阴道三维超声对子宫内膜息肉及子宫内膜息肉恶变诊断的应用研究 被引量:18

摘要 目的探讨经阴道三维超声对子宫内膜息肉及子宫内膜息肉恶变诊断的应用价值。方法选取2017年6月1日至2019年5月1日北京市垂杨柳医院收治的绝经1年以上,经阴道三维多普勒超声检查提示子宫内膜增厚或子宫内膜息肉或宫腔占位的患者159例,根据宫腔镜治疗后的病理结果,将其分为对照组(39例)、息肉组(98例)和恶变组(22例),分析3组患者临床特点及超声指标的差异。结果3组平均年龄和未产史比较,差异无统计学意义(P>0.05)。恶变组超重比例及绝经后出血发生率均明显高于息肉组[63.6%比27.6%,77.3%比24.5%],差异有统计学意义(P<0.05)。息肉组和恶变组的子宫内膜厚度均明显大于对照组[(9.83±3.81)mm和(11.64±5.87)mm比(5.19±2.55)mm],差异有统计学意义(P<0.05)。恶变组与息肉组相比,病灶更大[20(19.5,30.0)mm比10(6,14)mm],病灶回声不均匀比例[19例(86.4%)比44例(44.9%)]、病灶血流丰富比例[17例(77.3%)比14例(14.3%)]和阻力指数(resistance index,RI)<0.4的比例[16例(72.7%)比6例(6.1%)]更高,差异均有统计学意义(P<0.05)。结论阴道三维超声提示子宫内膜明显增厚、病灶较大、病灶回声不均匀及血流丰富,且RI<0.4时,对于子宫内膜息肉恶变具有一定的诊断价值。
