固肠止泻丸与蒙脱石散治疗腹泻型肠易激综合征患者的效果比较

Comparison of effects of Guchang Zhixie pills and Montmorillonite powder in treatment of patients with diarrhea-predominant irritable bowel syndrome

目的:比较固肠止泻丸与蒙脱石散治疗腹泻型肠易激综合征(IBS-D)患者的效果。方法:选取吉水县阜田镇中心卫生院收治的60例IBS-D患者为研究对象,按随机数字表法将患者分为对照组和观察组,各30例。对照组采用蒙脱石散治疗,观察组采用固肠止泻丸治疗,两组均持续治疗1个月。比较两组临床疗效、治疗前后血清胃肠激素指标[胃动素(MTL)、胃泌素(GAS)、生长抑素(SS)、血管活性肠肽(VIP)]水平、近端胃容积,以及不良反应发生率。结果:两组治疗总有效率比较,差异无统计学意义(P>0.05);治疗后,观察组MTL、GAS、SS、VIP水平均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组试餐后0、5、10、20、30 min近端胃容积均大于对照组,差异有统计学意义(P<0.05);试餐后60 min,两组近端胃容积比较,差异无统计学意义(P>0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:固肠止泻丸治疗IBS-D患者对腹痛、腹泻的临床疗效与蒙脱石散相当,但能够改善胃肠激素水平,增加近端胃容积。

Objective:To compare effects of Guchang Zhixie pills and Montmorillonite powder in treatment of patients with diarrheapredominant irritable bowel syndrome(IBS-D).Methods:60 patients with IBS-D who were admitted to the central health center of Futian town of Jishui county were selected as the research objects,and were divided into control group and observation group according to the random number table method,30 cases in each group.The control group was treated with Montmorillonite powder,while the observation group was treated with Guchang Zhixie pills.Both groups were continuously treated for 1 month.The clinical effects,the serum gastrointestinal hormone levels[motilin(MTL),gastrin(GAS),somatostatin(SS),vasoactive intestinal peptide(VIP)],the proximal gastric volume and the incidence of adverse reactions were compared between the two groups before and after treatment.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).After the treatment,the levels of MTL,GAS,SS and VIP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the proximal gastric volumes in the observation group were higher than those in the control group at 0,5,10,20,and 30 minutes after the trial meal,and the differences were statistically significant(P<0.05);however,there was no significant difference in the proximal gastric volume between the two groups 60 min after the trial meal(P>0.05).Further,here was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Guchang Zhixie ills in the treatment of the IBS-D patients can improve the levels of gastrointestinal hormones and increase the proximal gastric volume.Moreover,it is superior to Montmorillonite powder treatment.
