综合护理对急性胆囊炎腹腔镜切除术后病人恢复情况影响的Meta分析

Influence of comprehensive nursing on recovery of patients with acute cholecystitis after laparoscopic resection:a Meta-analysis

目的:评价综合护理在急性胆囊炎病人腹腔镜手术后的应用效果,为临床护理提供参考依据。方法:计算机检索PubMed、Web of Science、中国知网、万方数据库、维普数据库,检索时限为各数据库建库至2021年2月18日。经文献质量评价后,采用State 16.0软件进行Meta分析。结果:共纳入14篇文献,涉及1249例病人,其中对照组624例,试验组625例。Meta分析结果显示,试验组病人并发症发生率明显低于对照组[RR=0.27,95%CI(0.19,0.39),P<0.00001],首次排便时间[SMD=-2.01,95%CI(-2.30,-1.72),P<0.00001]、下床活动时间[SMD=-3.95,95%CI(-4.35,-3.54),P<0.00001]、住院时间[SMD=-2.16,95%CI(-2.35,-1.96),P<0.00001]短于对照组;护理满意度高于对照组[RR=1.15,95%CI(1.00,1.32),P=0.045],差异均有统计学意义。结论:现有证据表明,对急性胆囊炎腹腔镜手术病人术后实施综合护理能够加速康复进程,降低术后并发症发生率,提高病人的护理满意度。

Objective:To evaluate the application effect of comprehensive nursing in patients with acute cholecystitis after laparoscopic surgery,and to provide reference for clinical nursing.Methods:PubMed,Web of Science,CNKI,Wanfang Database,VIP Database were retrieved.The retrieval time limit was from the establishment of the database to February 18,2021.After the literature was assessed for quality,RevMan 5.3 software was used for meta-analysis.Results:A total of 14 articles were included,involving 1249 patients,including 624 in the control group and 625 in the experimental group.Meta-analysis results showed that the incidence of complications in the experimental group was significantly lower than that in the control group[RR=0.27,95%CI(0.19,0.39),P<0.00001],and the time to first defecation[SMD=-2.01,95%CI(-2.30,-1.72),P<0.00001],time to get out of bed[SMD=-3.95,95%CI(-4.35,-3.54),P<0.00001],length of hospital stay[SMD=-2.16,95%CI(-2.35,-1.96),P<0.00001]were shorter than that of the control group;nursing satisfaction was higher than that of the control group[RR=1.15,95%CI(1.00,1.32),P=0.045],the difference was statistically significant.Conclusion:Existing evidence indicates that comprehensive nursing for patients undergoing laparoscopic surgery for acute cholecystitis can accelerate the rehabilitation process,reduce the incidence of postoperative complications,and improve patient nursing satisfaction.
