综合康复训练在腓骨瓣修复下颌骨缺损病人中的应用

Application of comprehensive rehabilitation training in patients with mandibular defect repaired by fibular flap

目的:探讨综合康复训练在腓骨瓣修复下颌骨缺损术后病人中的应用效果。方法:选取2020年6月—2020年11月收治的腓骨瓣修复下颌骨缺损30例病人作为对照组,实施常规护理;选取2020年12月—2021年5月入院的32例病人作为观察组,在常规护理基础上进行综合康复训练,比较两组病人踝关节活动度、拇趾肌力、6 min步行距离(6MWT)、Tinetti平衡量表(POMA)评分、简明健康调查表(SF⁃36)、Enneking下肢功能评分和小腿肌间血栓丛形成(MCVT)情况。结果:术后3个月和6个月观察组病人踝关节活动度、拇趾肌力、6MWT、POMA评分、SF⁃36评分和Enneking下肢功能评分均优于对照组,差异有统计学意义(P<0.05);两组MCVT发生率比较差异有统计学意义(χ^(2)=5.363,P<0.05)。结论:腓骨瓣修复下颌骨缺损术后病人进行综合康复训练,可以有效预防MCVT,改善病人平衡能力和步态,促进下肢功能恢复,提高病人生活质量。

Objective:To explore the application effect of comprehensive rehabilitation training in patients with mandibular defects repaired by the fibular flap.Methods:A total of 30 patients with mandibular defect repaired by fibular flap treated from June 2020 to November 2020 were enrolled as the control group and received routine care;other 32 patients admitted to the hospital from December 2020 to May 2021 were enrolled as the observation group and received comprehensive rehabilitation training based on routine nursing.Compare the ankle joint mobility,hallux muscle strength,6⁃min walk test(6MWT),Performance Oriented Mobility Assessment(POMA),and The Short Form Health Survey(SF⁃36),Enneking grade for lower extremity function score,and muscular calf vein thrombosis(MCVT).Results:In the observation group,the ankle joint mobility,hallux muscle strength,6MWT,POMA score,SF⁃36 score,and Enneking grade score for lower extremity function were higher than those in the control group at 3 months,and 6 months after the operation,and the differences were statistically significant(P<0.05);difference in the incidence of MCVT between the two groups was statistically significant(χ^(2)=5.363,P<0.05).Conclusions:Comprehensive rehabilitation training in patients with mandibular defects repaired by fibular flap could effectively prevent MCVT,improve the patient's balance function and gait,promote the recovery of lower extremity function,and improve the patient's quality of life.
