β-受体阻滞剂对瓣膜置换术后病人心脏功能、心脏结构及神经内分泌激素的影响 被引量:2

摘要 目的观察β-受体阻滞剂对瓣膜置换术后病人心脏功能、心脏结构及神经内分泌激素的影响。方法选取2017年1月—2019年10月我院成功完成瓣膜置换术的病人100例,采用随机数字法分为对照组和观察组,各50例。对照组术后给予强心、利尿及血管紧张素转换酶抑制剂(ACEI)治疗,观察组在对照组基础上联合β-受体阻滞剂治疗。比较两组术前、术后2周、4周、8周及16周心脏功能及结构指标[左房内径(LAD)、左室舒张末期内径(LVEDD)、左室射血分数(LVEF)]、神经内分泌激素[心房利钠肽(ANP)、血管紧张素Ⅱ(AngⅡ)及醛固酮(ALD)]变化。结果两组术前LAD、LVEDD、LVEF、ANP、AngⅡ及ALD比较,差异均无统计学意义(P>0.05);术后2周、4周、8周及16周,两组LAD、LVEDD、ANP、AngⅡ及ALD均低于术前(P<0.05或P<0.01),术后16周LVEF均高于术前(P<0.05);观察组术后4周、8周及16周LAD、ANP、AngⅡ均低于对照组(P<0.05或P<0.01);两组术后2周、4周、8周及16周LVEDD、LVEF、ALD比较,差异无统计学意义(P>0.05)。结论β-受体阻滞剂可促进瓣膜置换术后病人神经内分泌激素水平下降,改善心脏功能及心脏结构。
