优化混合内镜黏膜下剥离术治疗大肠侧向发育型肿瘤的效果分析

Analysis of Effect of Optimized Hybrid Endoscopic Submucosal Dissection in the Treatment of Laterally Spreading Tumor

目的:研究优化混合内镜黏膜下剥离术(OHESD)治疗大肠侧向发育型肿瘤(LST)的临床效果。方法:连续性入选2020年8月-2022年1月九江市第一人民医院收治的基底直径>20 mm LST患者60例作为研究对象,根据随机数字表法将其分为研究组和对照组,每组30例。研究组采用OHESD治疗,对照组采用内镜黏膜下剥离术(ESD)治疗。比较两组手术时间、整块切除率、完整切除率、术中及术后出血量、并发症发生率。结果:研究组手术时间短于对照组,术中及术后出血量均少于对照组,整块切除率、完整切除率均高于对照组,并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论:OHESD治疗LST的效果与传统ESD相当,手术操作更简易,创伤小,并发症少,更有利于患者术后恢复。

Objective:To study the clinical effect of optimizing hybrid endoscopic submucosal dissection(OHESD)in the treatment of laterally spreading tumor(LST).Method:A total of 60 patients with LST with a basal diameter>20 mm who admitted to Jiujiang NO.1 People’s Hospital from August 2020 to January 2022 were consecutively selected as the study objects,and they were divided into the study group and the control group according to the random number table method,30 cases in each group.The study group was given OHESD,while the control group was given traditional endoscopic submucosal dissection(ESD).The operation time,total resection rate,complete resection rate,intraoperative and postoperative bleeding and complication rate were compared between two groups.Result:The operation time of the study group was shorter than that of the control group,the intraoperative and postoperative blood loss were less than those of the control group,the enbloc resection rate and complete resection rate were higher than those of the control group,the complication rate was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion:The efficacy of OHESD in the treatment of LST is comparable to that of the traditional ESD,the operation is easier,less trauma,less complications and lower recurrence rate,it is beneficial to patient’s recovery after surgery.
