宫腔镜下不同手术方式治疗子宫内膜息肉的临床疗效对比

Comparison on the clinical efficacy of different hysteroscopic surgical methods in the treatment of endometrial polyps

目的探讨宫腔镜下不同手术方式治疗子宫内膜息肉的临床疗效。方法90例子宫内膜息肉患者,依据双盲法分为参照组与研究组,各45例。参照组实施宫腔镜刮宫术治疗,研究组实施宫腔镜刨削术治疗。对比两组远期并发症发生情况,临床指标,手术前后的子宫内膜厚度。结果研究组远期并发症发生率0低于参照组的8.89%,差异有统计学意义(P<0.05)。研究组术中出血量(3.8±0.4)ml少于参照组的(15.0±0.7)ml,手术时间(13.7±1.3)min及住院时间(4.2±0.8)d均短于参照组的(28.5±2.2)min、(6.5±1.5)d,差异有统计学意义(P<0.05)。术后3、6个月,两组患者子宫内膜厚度均优于本组术前,且研究组均优于参照组,差异均有统计学意义(P<0.05)。结论子宫内膜息肉患者采用宫腔镜刨削术进行治疗的效果更加理想,不仅对患者的创伤更小,还能够改善患者的身体状态,并发症少,值得在临床上大力推广。

Objective To discuss the clinical efficacy of different hysteroscopic surgical methods in the treatment of endometrial polyps.Methods A total of 90 patients with endometrial polyps were divided into reference group and research group according to a double-blind method,with 45 cases in each group.The reference group was treated with hysteroscopic curettage,and the research group was treated with hysteroscopic electrotomy.Both groups were compared in terms of long-term complications,clinical indicators,and endometrial thickness before and after surgery.Results The long-term complication rate of the research group was,which was 0 lower than 8.89%of the reference group,and the difference was statistically significant(P<0.05).The intraoperative blood loss(3.8±0.4)ml in the research group was less than(15.0±0.7)ml in the reference group;the operation time and hospitalization time were(13.7±1.3)min and(4.2±0.8)d in the research group,which were shorter than(28.5±2.2)min and(6.5±1.5)d in the reference group;all the differences were statistically significant(P<0.05).At 3 and 6 months after surgery,the endometrial thickness of the two groups was better than that before surgery,and the research group was better than the reference group,and the differences were statistically significant(P<0.05).Conclusion Hysteroscopic electrotomy has a more ideal effect for patients with endometrial polyps,which not only reduces the trauma to patients,but also improves the physical state of patients and prevents complications in patients.Therefore,it is worth vigorously promoting in clinical practice.
