摘要 目的研究高频超声诊断在小儿发育性髋关节发育不良(DDH)中的作用。方法选取100例X线检测确诊为小儿DDH的患儿作为实验组,同期100例X线检测为非DDH的小儿作为参照组。两组患儿均使用高频超声进行诊断。比较两组小儿基线与髋臼盖线夹角(α角)与基线与软骨髋臼盖线夹角(β角),以及记录使用高频超声诊断小儿DDH检出情况。结果实验组α角(39.78±5.12)°显著小于参照组的(62.37±6.13)°,β角(89.77±12.36)°显著大于参照组的(48.61±5.51)°,差异均具有统计学意义(P<0.05)。X线检查结果:实验组100例小儿DDH患儿中有73例髋关节脱位和27例髋臼发育不良。高频超声诊断:有96例诊断为小儿DDH,其中男30例,女66例;71例髋关节脱位和25例髋臼发育不良。高频超声诊断小儿DDH检出率为96%。结论高频超声诊断小儿DDH的检出率较高,可实施于小儿DDH的早期筛查中。 Objective To study the role of high frequency ultrasound in the diagnosis of developmental dysplasia of the hip(DDH)in children.Methods 100 children diagnosed with DDH by X-ray were selected as the experimental group,and 100 children with non-DDH by X-ray during the same period were selected as the reference group.High-frequency ultrasound was used for diagnosis in both groups.The differences between theαangle(angle between baseline and line of acetabulum)andβangle(angle between baseline and inclination line)were compared between the two groups,and the detection of DDH in children was recorded using high-frequency ultrasound diagnosis.Results Theαangle(39.78±5.12)°of the experimental group was significantly less than(62.37±6.13)°of the reference group,and theβangle(89.77±12.36)°was significantly greater than(48.61±5.51)°of the reference group.All the differences were statistically significant(P<0.05).According to the X-ray examination,there were 73 cases of hip dislocation and 27 cases of acetabular dysplasia among 100 children with DDH in the experimental group.After high-frequency ultrasound diagnosis,96 cases were diagnosed with DDH,including 30 males and 66 females,with 71 cases of hip dislocation and 25 cases of acetabular dysplasia.The detection rate of children in the experimental group by high-frequency ultrasound was 96%.Conclusion The diagnosis rate of high frequency ultrasound is high,which can be used in the early screening of children with DDH.
