补中益气汤加减联合他汀类药物治疗老年冠心病合并慢性心力衰竭的临床效果

Clinical effect of modified Buzhong Yiqi decoction combined with statins in the treatment of elderly patients with coronary heart disease complicated with chronic heart failure

目的分析补中益气汤加减联合他汀类药物治疗老年冠心病合并慢性心力衰竭的临床疗效。方法68例老年冠心病合并慢性心力衰竭患者,随机分为观察组与对照组,各34例。对照组给予阿托伐他汀治疗,观察组给予补中益气汤加减联合阿托伐他汀治疗。比较两组治疗效果、治疗前后心功能、运动耐力及中医证候积分。结果观察组治疗总有效率94.12%高于对照组的73.53%,差异有统计学意义(P<0.05)。治疗后,观察组左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)、6 min步行距离(6MWD)分别为(54.25±5.32)mm、(40.03±3.53)mm、(50.07±5.03)%、(338.26±68.37)m,均优于对照组的(57.57±5.74)mm、(42.63±4.25)mm、(43.12±4.43)%、(277.42±65.42)m,差异均有统计学意义(P<0.05)。治疗后,观察组自汗、疲倦乏力、心悸、气短评分分别为(1.80±0.86)、(2.20±0.84)、(2.24±0.90)、(2.64±1.02)分,均低于对照组的(2.64±1.81)、(3.58±0.67)、(3.30±1.41)、(3.35±1.41)分,差异均有统计学意义(P<0.05)。结论补中益气汤加减联合他汀类药物治疗老年冠心病慢性心力衰竭,能够有效改善患者的心功能、运动耐力和临床症状,从而显著提升其临床疗效。

Objective To analyze the clinical efficacy of modified Buzhong Yiqi decoction combined with statins in the treatment of elderly patients with coronary heart disease complicated with chronic heart failure.Methods A total of 68 elderly patients with coronary heart disease and chronic heart failure were randomly divided into observation group and control group,with 34 cases in each group.The control group was treated with atorvastatin,and the observation group was treated with modified Buzhong Yiqi Decoction and atorvastatin.The therapeutic effect,cardiac function,exercise tolerance and traditional Chinese medicine syndrome scores before and after treatment were compared between the two groups.Results The total effective rate 94.12%of the observation group was higher than 73.53%of the control group,and the difference was statistically significant(P<0.05).After treatment,the left ventricular end-diastolic diameter(LVEDD),left ventricular endsystolic diameter(LVESD),left ventricular ejection fraction(LVEF),and 6-min walking distance(6MWD)in the observation group were(54.25±5.32)mm,(40.03±3.53)mm,(50.07±5.03)%,and(338.26±68.37)m,which were better than(57.57±5.74)mm,(42.63±4.25)mm,(43.12±4.43)%,and(277.42±65.42)m in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of spontaneous sweating,fatigue,palpitations and shortness of breath in the observation group were(1.80±0.86),(2.20±0.84),(2.24±0.90),and(2.64±1.02)points,which were all lower than(2.64±1.81),(3.58±0.67),(3.30±1.41),and(3.35±1.41)points in the control group,and the differences were statistically significant(P<0.05).Conclusion Modified Buzhong Yiqi decoction combined with statins for elderly patients with chronic heart failure and coronary heart disease can effectively improve the patients'cardiac function,exercise tolerance and clinical symptoms,thereby significantly improving their clinical efficacy.
