半夏泻心汤联合兰索拉唑治疗消化道溃疡的疗效观察

Effect of BanxiaXiexin Decoction Combined with Lansoprazole on Peptic Ulcer

目的探讨半夏泻心汤联合兰索拉唑治疗消化道溃疡的疗效观察。方法选取2017年1月—2020年12月期间于安徽省淮北矿工总医院就诊的消化道溃疡患者110例,按随机数字表法分为兰索拉唑组和联合中医组,每组各55例。兰索拉唑组使用兰索拉唑治疗,联合中医组采用半夏泻心汤联合兰索拉唑治疗。治疗1周后,观察比较两组患者临床疗效、症状缓解时间,治疗前后肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、三叶因子3(Trefoil factor 3,TFF3)、表皮生长因子(Epidermal Growth Factor,EGF)水平,胃泌素、胃动素变化及pH值、血浆纤维蛋白、血清D-D水平。结果两组患者治疗后腹痛症状、腹胀症状、烧心不适缓解时间均优于兰索拉唑组,两组比较,差异有统计学意义(P<0.05)。治疗后两组患者TNF-α、TFF3、EGF水平均较治疗前明显降低,差异有统计学意义(P<0.05);且联合中医组TNF-α、TFF3、EGF水平明显低于兰索拉唑组,差异有统计学意义(P<0.05)。治疗后两组患者胃泌素、胃动素水平均较治疗前降低,差异有统计学意义(P<0.05);且联合中医组胃泌素、胃动素水平明显低于兰索拉唑组,差异有统计学意义(P<0.05)。治疗后两组患者pH值均较治疗前升高,血浆纤维蛋白、血清D-D水平均较治疗前下降,差异有统计学意义(P<0.05);且联合中医组pH值明显高于兰索拉唑组,血浆纤维蛋白、血清D-D水平明显低于兰索拉唑组,差异有统计学意义(P<0.05)。治疗后联合中医组总有效率为92.73%(51/55),兰索拉唑组为72.72%(40/55),两组比较,差异有统计学意义(P<0.05)。结论半夏泻心汤联合兰索拉唑治疗消化道溃疡可有效降低患者疼痛,减少症状缓解时间,降低患者组织损伤,抑制炎症介质增高,提高治疗效果。

Objective To explore the efficacy of BanxiaXiexinDecoction combined with lansoprazole inthe treatment of peptic ulcer.Methods A total of 110 patients with peptic ulcer treated in Huaibei Miners General Hospitalfrom January 2017 to December 2020 were randomly assigned to a control group(lansoprazole)and anobservation group(lansoprazole+BanxiaXiexinDecoction),with 55 patients ineachgroup.After one week of treatment,the time tosymptom relief and the clinical efficacy were compared between the two groups.The tumor necrosis factor-alpha(TNF-α),trefoil factor 3(TFF3),epidermal growth factor(EGF),gastrin,motilin,pH,plasma fibrin,and serum D-D levels were compared between before treatment and after treatment as well as between the two groups.Results The observation group had shorter time to abdominal pain relief,abdominal distension relief,and heartburn relief than the control group(P<0.05).After treatment,the TNF-α,TFF3,and EGF levels,as well as the gastrin and motilin levels in both groupsdeclined compared with those before treatment and were lower in the observation group than in the control group(P<0.05).The treatments in both groups elevated the pH value and reduced the plasma fibrin and serum D-D levels(P<0.05).Moreover,the observation group had higher pH value and lower plasma fibrin and serum D-D levels than the control group(P<0.05).After treatment,the observation group had higher total effective rate[92.73%(51/55)vs.72.72%(40/55),P<0.05]than the control group.Conclusion BanxiaXiexinDecoction combined with lansoprazole can mitigate the pain,shorten the time to symptom relief,reduce the tissue damage,and inhibit the elevation of inflammatory cytokines,demonstrating remarkable therapeutic effect on peptic ulcer.
