外周血T细胞白细胞相关免疫球蛋白样受体1的表达与宫颈上皮内瘤变Ⅲ级及宫颈癌患者高危型人乳头瘤病毒感染的关系

Relationships between LAIR-1 expression on peripheral blood T cells and HR-HPV infection in patients with CINⅢand cervical cancer

目的研究外周血T细胞白细胞相关免疫球蛋白样受体1(LAIR-1)的表达与宫颈上皮内瘤变Ⅲ级(CINⅢ)和宫颈癌患者高危型人乳头瘤病毒(HR-HPV)感染的相关性。方法回顾性分析海口市妇幼保健院2017年1月至2020年3月收治的92例CINⅢ患者(A组)、87例宫颈癌患者(B组)、90例健康志愿者(对照组)的相关资料,对比三组外周血T细胞LAIR-1的表达,统计A组和B组HPV的感染情况,对比A组和B组中HR-HPV感染者与无HR-HPV感染者外周血T细胞LAIR-1的表达,分析A组和B组外周血T细胞LAIR-1的表达与HR-HPV感染的关系。结果对照组的CD3^(+)CD4^(+)T细胞LAIR-1的表达明显较A、B两组低,同时B组的表达显著高于A组(P<0.05);A、B组的CD3^(+)CD8^(+)T细胞LAIR-1表达与对照组比较无明显差异(P>0.05)。B组多重感染、双重感染和HR-HPV单一感染率明显高于A组,同时A组患者的HPV16、HPV18、HPV52感染率均低于B组(P<0.05)。A、B组HR-HPV感染者中的CD3^(+)CD4^(+)T细胞LAIR-1表达明显高于未感染HR-HPV者(P<0.05);A、B组未感染HR-HPV者的CD3^(+)CD8^(+)T细胞LAIR-1表达与HR-HPV感染者比较无明显差异(P>0.05)。A组和B组CD3^(+)CD4^(+)T细胞LAIR-1的表达与HR-HPV感染均呈正相关(r=0.741、0.803,P=0.012、0.005),CD3^(+)CD8^(+)T细胞LAIR-1的表达与HR-HPV感染均无相关性(P>0.05)。结论CINⅢ和宫颈癌患者CD3^(+)CD4^(+)T细胞LAIR-1的表达与HR-HPV感染呈正相关性,且CD3^(+)CD4^(+)T细胞LAIR-1的表达可作为病情分级的预测指标。

Objective To analyze the relationship between the expression of leukocyte associated immunoglobulin like receptor 1(LAIR-1)on peripheral blood T cells and high-risk human papilloma virus(HR-HPV)infection in patients with cervical intraepithelial neoplasia gradeⅢ(CINⅢ)and cervical cancer.Methods The data of 92 patients with CINⅢ,87 patients with cervical cancer and 90 healthy volunteers were retrospectively analyzed,who were recorded as group A,group B and control group.The expressions of LAIR-1 in peripheral blood T cells were compared among the three groups.The HPV infection situations in group A and group B were statistically analyzed,and the expressions of LAIR-1 in peripheral blood T cells of patients with HR-HPV infection or not were compared in group A and group B.The relationships between the expressions of LAIR-1 in peripheral blood T cells and HR-HPV infection in group A and group B were analyzed.Results The expression of CD3^(+)CD4^(+)T cells LAIR-1 in control group was significantly lower than that in group A and B,that in group B higher than group A(P<0.05);there′s no difference between the expression of CD3^(+)CD8^(+)T cells in group A and B and control group(P>0.05).The rates of multiple infection,double infection and HR-HPV single infection in group B were significantly higher than those in group A,while the rates of HPV16,HPV18 and HPV52 infection in group A were all lower than those in group B(P<0.05).The expression of CD3^(+)CD4^(+)T cells LAIR-1 in HR-HPV infected group A and B was significantly higher than that in non-infection group(P<0.05).The expression of CD3^(+)CD4^(+)T cells LAIR-1 in patients without HR-HPV infection was not significantly different from that in patients with HR-HPV infection(P>0.05).The expression of CD3^(+)CD4^(+)T cells LAIR-1 in group A and group B was positively correlated with HR-HPV infection(r=0.741,0.803,P=0.012,0.005),while there was no correlation between the expression of CD3^(+)CD4^(+)T cells LAIR-1 and HR-HPV infection(P>0.05).Conclusions The expression of CD3^(+)CD4^(+)T cell LAIR-1 in patients with CINⅢand cervical cancer was positively correlated with HR-HPV infection,and CD3^(+)CD4^(+)T cell LAIR-1 could be used as a predictor of disease grade.
