6岁以下儿童血清总免疫球蛋白E、呼出气一氧化氮浓度、外周血嗜酸性粒细胞水平与哮喘的关系

摘要 目的:探讨血清总免疫球蛋白E(tIgE)、呼出气一氧化氮浓度(FeNO)、外周静脉血嗜酸性粒细胞(EOS)计数在6岁以下喘息儿童中的应用价值。方法:选取儿科住院的6岁以下的下呼吸道感染患者共100例作为研究对象。根据是否有喘息分为喘息组(n=65)及非喘息组(n=35)。非喘息组患者随诊18~55个月,无诊断哮喘患者。喘息组患者随诊18~55个月,根据最终是否发展为哮喘分为哮喘组(n=31)与非哮喘组(n=34)。所有患者入院时采静脉血,检测血清总IgE水平,FeNO水平,EOS水平。结果:比较非喘息组与喘息组患者外周血EOS计数、FeNO水平,差异无统计学意义(P>0.05),喘息组tIgE高于非喘息组,差异有统计学意义(P<0.05)。喘息患者最终诊断哮喘组与非哮喘组外周血EOS计数、tIgE、FeNO比较差异无统计学意义(P>0.05)。结论:6岁以下喘息患者中高tIgE水平,发展为哮喘可能性大,6岁以下喘息患者外周血嗜酸性粒细胞、FeNO水平不能作为预测哮喘指标。 Objective To explore the application value of serum total immunoglobulin E(tIgE), fractional exhaled nitric oxide(FeNO) concentration, peripheral venous blood eosinophil(EOS) level in children with wheezing under 6 years old.Method A total of 100 patients with lower respiratory tract infection under 6 years old who were hospitalized in the pediatrics department were selected as the research objects. According to whether there was wheezing or not, they were divided into wheezing group(n=65) and non-wheezing group(n=35). Patients in the non-wheezing group were followed up for 18 to 55 months, and no asthma patients were diagnosed.Patients in the wheezing group were also followed up for 18 to 55 months, they were divided into asthma group(n=31) and non-asthmatic group(n=34) according to whether they developed asthma. Venous blood was collected from all patients when they were hospitalized. The serum total IgE level, EOS level and FeNO level were detected.Results The peripheral blood EOS level and FeNO level of the non-wheezing group and the wheezing group were compared, and the difference was not statistically significant( P>0.05),the tIgE of the wheezing group was higher than that of the non-wheezing group, with a statistically significant difference(P<0.05). Additionally, there was no significant significance in the peripheral blood EOS level, tIgE, and FeNO level between the asthma group and the non-asthma group(P>0.05).Conclusion Patients with wheezing under 6 years old presenting high tIgE levels are more likely to develop asthma. Peripheral blood EOS level and FeNO level in wheezing patients under 6 years old cannot be used as predictive indicators of asthma.
