血清血管内皮生长因子、癌胚抗原、CA153、CA125及CA199检测在乳腺癌不同形式化疗中的意义 被引量:4

摘要 目的:探讨血清血管内皮生长因子(VEGF)、癌胚抗原(CEA)、CA153、CA125及CA199检测在乳腺癌不同形式化疗中的意义。方法:选取2020年1月~2020年10月112例乳腺癌化疗患者作为观察组,其中新辅助化疗患者32例,辅助化疗患者50例,解救化疗患者30例,并以同期乳腺良性病变患者45例为对照组。检测观察组及对照组患者初始血清VEGF、CEA、CA153、CA125及CA199及观察组患者进行2个疗程化疗后各项指标水平。比较对照组、观察组治疗前上述指标差异及观察组化疗前后各项指标变化情况。结果:新辅助化疗组、解救化疗组患者初始血清VEGF、CEA、CA153水平显著高于对照组及辅助化疗组,差异均有统计学意义(P<0.05),且新辅助化疗组显著高于解救化疗组(P<0.05),而辅助化疗组与对照组相比,差异无统计学意义(P>0.05);对于血清CA125、CA199水平,除新辅助化疗组CA199明显升高,其余各组间差异均无统计学意义(P>0.05)。经2个疗程化疗后,新辅助化疗组及解救化疗组患者血清VEGF、CEA、CA153均下降,差异有统计学意义(P<0.05),辅助化疗组差异无统计学意义(P>0.05);对于血清CA125、CA199水平,各组患者化疗前后差异均无统计学意义(P>0.05)。结论:本文所研究的肿瘤标志物中,血清VEGF、CA153及CEA检测或可作为肿瘤负荷存在情况下乳腺癌化疗后疗效监测和评估的重要指标。 Objective To investigate the significance of serum vascular endothelial growth factor(VEGF), carcinoembryonic antigen(CEA), CA153, CA125 and CA199 in different forms of chemotherapy for breast cancer.Method 112 cases of breast cancer patients undergoing chemotherapy admitted to our hospital from January to October 2020 were selected as the observation group, including 32 cases of neoadjuvant chemotherapy(NC)patients, 50 cases of adjuvant chemotherapy(AC) patients, 30 cases of rescue chemotherapy(RC) patients, and 45 cases of patients with benign breast lesions admitted to our hospital during the same period as the control group. The initial serum VEGF, CEA, CA153, CA125 and CA199 in observation group and control group were detected, as well as these indexes in observation group after 2 courses of chemotherapy. The differences of above indexes between the control group and the observation group before treatment and the changes of indexes before and after chemotherapy in the observation group were compared.Results The initial serum levels of VEGF, CEA and CA153 in the neoadjuvant chemotherapy group and rescue chemotherapy group were significantly higher than those in the control group(P<0.05) and the adjuvant chemotherapy group(P<0.05), and the neoadjuvant chemotherapy group was significantly higher than that in the rescue chemotherapy group(P<0.05), while there was no statistical significance between the adjuvant chemotherapy group and the control group(P>0.05);as for serum CA125 and CA199 levels, there was no statistical significance among all groups except that CA199 was significantly increased in the neoadjuvant chemotherapy group(P<0.05). After 2 courses of chemotherapy, serum VEGF, CEA and CA153 decreased in both the neoadjuvant chemotherapy group and the rescue chemotherapy group, with statistical significance(P<0.05), while there was no statistical significance in the adjuvant chemotherapy group(P>0.05). There was no statistical significance in serum CA125 and CA199 levels before and after chemotherapy in all groups(P>0.05).Conclusion Among the tumor markers studied in this study, serum VEGF, CA153 and CEA may be used as important indicators for monitoring and evaluating the efficacy of breast cancer after chemotherapy in the presence of tumor load.
