首发未服药抑郁症患者的额叶α不对称性 被引量:2

The Frontal Alpha Asymmetry in Patients with First-episode Drug-na?ve Major Depressive Disorder

摘要 目的:考察抑郁症(Major Depressive Disorder,MDD)患者静息态脑电(Resting-State Electroencephalogram,rsEEG)中的额叶α不对称性(Frontal Alpha Asymmetry,FAA)能否作为有效识别抑郁症的神经电生理标记。方法:42名首发未服药MDD患者和42名健康对照被试(Health Control,HC)完成了rsEEG的采集,以及流调中心用抑郁量表、状态焦虑量表、简明情感强度量表负性情感强度分量表的评估。采用重复测量方差分析比较两组在功率谱密度(Power Spectral Density,PSD)与FAA上的差异,采用机器学习算法进行特征筛选与分类验证,并计算所筛选特征与心理量表的相关程度。结果:各电极对中,MDD组右侧电极的PSD均显著小于左侧电极,HC组右侧电极的PSD值均显著大于左侧电极。MDD组的FAA值均显著低于HC组。FAA_(F4-F3)、FAA_(F6-F5)、FAA_(F8-F7)可有效识别MDD,且与抑郁、焦虑、负性情感强度量表得分均呈显著负相关。结论:静息状态下,MDD患者右侧额叶的活动强度较左侧额叶更强,其额叶不对称性表现出与健康个体相反的模式,这可能是MDD潜在的神经电生理标记之一。 Objective:To examine whether the abnormal frontal alpha asymmetry(FAA) in major depressive disorder(MDD) patients during resting state electroencephalogram(rsEEG) could be neurophysiological markers to identify MDD.Methods:Forty-two first-episode,drug-na?ve MDD patients and 42 gender-,age-,and intelligence-matched healthy controls(HCs) completed rsEEG,the Center for Epidemiologic Studies Depression Scale(CES-D),the State Anxiety Inventory(SAI),and the Short Affect Intensity Scale-Negative Subscale(SAIS_N).The repeated measurement analysis of variance was adopted to compare the differences in power spectral density(PSD) and FAA between MDD and HC group.The machine learning algorithm was conducted to feature screening and classification validation,and the correlations between the screened features and the psychological variables were calculate.Results:(1)The PSD of the right electrode was significantly smaller than that of the left electrode in the MDD group,and the PSD values of the right electrode were significantly larger than that of the left electrode in the HC group.(2)The FAA value of MDD group was significantly lower than HC group.(3)The FAA_(F4-F3),FAA_(F6-F5),and FAA_(F8-F7) could effectively identify MDD patients.(4)The FAA_(F4-F3),FAA_(F6-F5),and FAA_(F8-F7) were negatively correlated with the scores of the CES-D,SAI,and SAIS_N.Conclusion:In the resting state,the activity intensity of the right frontal is stronger than that of the left frontal in MDD patients.The opposite pattern of frontal alpha asymmetry to general population may be one of the potential neurophysiological markers of MDD.
