益气养阴汤联合常规疗法治疗慢性心力衰竭稳定期临床研究

Clinical Study on Yiqi Yangyin Tang Combined with Routine Therapy for Chronic Heart Failure in Stable Stage

目的:观察益气养阴汤联合常规疗法治疗慢性心力衰竭稳定期的临床效果以及预后。方法:选择82例慢性心力衰竭稳定期患者,按随机数字表法分为对照组和研究组各41例。对照组给予常规西医对症治疗,研究组在对照组基础上联合益气养阴汤治疗,2组均连续治疗6个月。评价2组临床疗效,比较2组治疗前后左心室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室收缩末期容积(LVESV),采用明尼苏达心力衰竭生活质量量表(MLHFQ)评价患者生活质量,计算2组6 min步行(6MWT)距离、再住院率、死亡率。结果:研究组总有效率为92.68%,对照组为80.49%,研究组总有效率高于对照组(P<0.05)。治疗后,2组LVEF水平较治疗前升高(P<0.05),研究组LVEF水平高于对照组(P<0.05);2组LVESD、LVESV水平较治疗前降低(P<0.05),研究组LVESD、LVESV水平低于对照组(P<0.05)。治疗后,2组6MWT水平较治疗前升高(P<0.05),研究组6MWT水平高于对照组(P<0.05);2组MLHFQ水平较治疗前降低(P<0.05),研究组MLHFQ水平低于对照组(P<0.05)。研究组再住院率为14.63%,低于对照组36.59%,差异有统计学意义(P<0.05)。结论:益气养阴汤联合常规疗法治疗慢性心力衰竭稳定期患者,可改善患者心功能,提升步行距离及生活质量,降低患者再住院率,提高临床疗效,改善预后。

Objective:To observe the clinical effect and prognosis of Yiqi yangyin tang combined with routine therapy for chronic heart failure in stable stage.Methods:A total of 82 cases of patients with chronic heart failure in stable stage were selected and divided into the control group and the study group according to the random number table method,with 41 cases in each group.The control group was given symptomatic treatment of routine western medicine,and the study group was additionally treated with Yiqi Yangyin tang based on the treatment of the control group.Both groups were treated for six months.The clinical effects in the two groups were evaluated.Before and after treatment,left ventricular ejection fraction(LVEF),left ventricular end-systolic dimension(LVESD)and left ventricular end-systolic volume(LVESV)in the two groups were compared.The quality of life was evaluated by Minnesota Living with Heart Failure Questionnaire(MLHFQ).The sixminute walk test(6MWT)distance,readmission rate and mortality in the two groups were counted.Results:The total effective rate was 92.68%in the study group and 80.49%in the control group,and the one in the study group was higher than that in the control group(P<0.05).After treatment,the LVEF levels in the two groups were increased when compared with those before treatment(P<0.05),and the LVEF level in the study group was higher than that in the control group(P<0.05);the levels of LVESD and LVESV in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of LVESD and LVESV in the study group were lower than those in the control group(P<0.05).After treatment,the 6MWT levels in the two groups were increased when compared with those before treatment(P<0.05),and the 6MWT level in the study group was higher than that in the control group(P<0.05);the MLHFQ levels in the two groups were decreased when compared with those before treatment(P<0.05),and the MLHFQ level in the study group was lower than that in the control group(P<0.05).The readmission rate was 14.63%in the study group,lower than that of 36.59%in the control group,the difference being significant(P<0.05).Conclusion:Yiqi yangyin tang combined with routine therapy for chronic heart failure in stable stage can improve heart function,walking distance,quality of life,clinical effect and prognosis,and decrease the readmission rate.
