消银胶囊辅助治疗寻常型银屑病血热证临床研究

Clinical Study on Xiaoyin Capsules for Psoriasis Vulgaris with Blood Heat Syndrome

目的:观察消银胶囊辅助治疗寻常型银屑病血热证的临床疗效及对银屑病皮损面积与严重程度指数(PASI)评分、皮肤屏障功能的影响。方法:选取寻常型银屑病血热证患者108例,按随机数字表法分为观察组和对照组各54例。对照组给予郁金消银汤治疗,观察组在对照组基础上联合消银胶囊治疗。比较2组治疗前后炎性因子[白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-10(IL-10)、γ-干扰素(IFN-γ)]、PASI评分和皮肤屏障功能变化,评价临床疗效和不良反应发生情况。结果:观察组总有效为96.30%,高于对照组83.33%,差异有统计学意义(P<0.05)。治疗后,2组PASI评分及IFN-γ水平较治疗前降低,且观察组PASI评分及IFN-γ水平均低于对照组(P<0.05)。治疗后,2组血清IL-2、IL-4、IL-10水平低于治疗前,且观察组血清IL-2、IL-4、IL-10水平均低于对照组(P<0.05)。治疗后,2组经皮水分丢失(TEWL)值较治疗前降低,且观察组TEWL值低于对照组(P<0.05);2组角质层含水量和皮脂含量较治疗前升高,且观察组角质层含水量和皮脂含量高于对照组(P<0.05)。结论:消银胶囊辅助治疗寻常型银屑病血热证临床疗效理想,可有效改善患者的皮损程度,降低炎性因子水平,提高皮肤屏障功能,治疗过程中未增加不良反应。

Objective:To observe the clinical effect of Xiaoyin capsules on the adjuvant treatment of psoriasis vulgaris with blood heat syndrome and its effect on the score of Psoriasis Area and Severity Index(PASI)and skin barrier function.Methods:A total of 108 patients with psoriasis vulgaris with blood heat syndrome were selected and divided into the observation group and the control group according to random number table method,with 54 cases in each group.The control group was treated with Yujin Xiaoyin tang,and the observation group was additionally treated with Xiaoyin capsules based on the treatment of the control group.The changes of inflammatory factors,including interleukin-2(IL-2),interleukin-4(IL-4),interleukin-10(IL-10)andγ-interferon(IFN-γ),the PASI score and skin barrier function in both groups were compared before and after treatment,and the clinical effects and incidence of adverse reactions were evaluated.Results:The total effective rate was 96.30%in the observation group,higher than that of 83.33%in the control group,the difference being significant(P<0.05).After treatment,the PASI scores and the IFN-γlevels in both groups were decreased when compared with those before,and the PASI score and the IFN-γlevel in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of serum IL-2,IL-4 and IL-10 in the two groups were lower than those before treatmen,and the above levels in the observation group were lower than those in the control group(P<0.05).After treatment,the TEWL values in the two groups were decreased when compared with those before treatment,and the TEWL value in the observation group was lower than that in the control group(P<0.05).The corneous water content and sebum content in the two groups were increased when compared with those before treatment,and the corneous water content and sebum content in the observation group were higher than those in the control group(P<0.05).Conclusion:Xiaoyin capsules has an ideal clinical effect on the adjuvant treatment of psoriasis vulgaris with blood heat syndrome,which can effectively improve the degree of skin lesions,reduce the levels of inflammatory factors and improve the skin barrier function,without increasing the adverse reactions in the treatment process.
