摘要
目的:探讨3D虚拟现实模拟培训对外科医师学习腹腔镜胆囊切除术的作用。方法:外科医师34名随机分两组,干预组参加虚拟现实培训,对照组参加传统腹腔镜临床培训。培训完成后,34名学员分别在高年资医师的监督下完成3例腹腔镜胆囊切除术。手术录像随机交于评分医师打分。比较两组手术完成率、手术评分、手术时间等。结果:干预组3次手术完成率分别为76.5%、88.2%、94.1%;对照组3次手术完成率分别为47.1%、70.6%、82.4%;干预组3次手术总完成率(88.3%)较对照组(66.7%)更高,差异有统计学意义(χ^(2)=5.449,P<0.05);干预组3次手术平均评分为61.71±7.372,高于对照组(46.88±5.555),差异有统计学意义(t=6.621,P<0.05);干预组3次手术平均时间为(77.53±2.267)min,对照组为(82.88±2.261)min,两组比较差异有统计学意义(t=-6.894,P<0.05)。参加3D虚拟现实模拟培训医师的手术完成率、手术评分、手术时间均优于传统腹腔镜技能培训组。结论:3D虚拟现实模拟培训可以加快青年外科医师腹腔镜胆囊切除术理论基础向临床操作技能的转化。
Objective:A randomized controlled study was conducted to explore the role of virtual reality simulation training in the growth of surgeons by comparing the performance of young doctors in laparoscopic cholecystectomy.Methods:Thirty-four young doctors were randomly divided into two groups.The intervention group took part in virtual reality simulation training and the control group took part in traditional laparoscopic clinical training.After the training,34 students completed 3 cases of laparoscopic cholecystectomy under the supervision of senior doctors.The surgical video was randomly handed over to the scoring physician.The data of operation completion rate,operation score and operation time were recorded and analyzed statistically.Results:The three surgical completion rates of the intervention group and the control group were 76.5%>47.1%,88.2%>70.6%,and 94.1%>82.4%.The total completion rate of the 2 groups of 3 operations was88.3%>66.7%(χ^(2)=5.449,P<0.05).The total scores of the operation group and the control group were 67.71±7.372,53.88±5.555(t=6.621,P<0.05).The average time of the 3 operations in the 2groups was 77.53±2.267min,82.88±2.261min(t=-6.894,P<0.05),there were statistical significance.Conclusion:The completion rate,surgical score and operation time of the students participating in virtual reality simulation training are better than those of the traditional laparoscopic skill training group.3D virtual reality simulation training can transfer the basic skills of laparoscopic training into clinical operation skills,and can promote the growth of young surgeons.
