摘要
目的探讨尿液中23种金属浓度与精液质量参数的关联性。方法招募394名育龄男性,收集精液和尿液样本,使用电感耦合等离子体质谱联用仪(inductively coupled plasma-mass spectrometry,ICP-MS)检测尿液中23种金属浓度,对每种尿液金属浓度四分位数分组(<P25组、P25~组、P50~组、P75~组),运用多重线性回归分析探讨尿液金属不同分组浓度与精液质量参数的关联性。结果尿锌、尿锑、尿铅浓度与精子质量参数之间存在显著的剂量-反应关系(均P_(trend)<0.05)。与尿锌浓度第一分位数组相比,精子前向运动活力(%)(经平方根反正弦变换后)、精子总活力、精子存活率(%)(经平方根反正弦变换后)分别在第四分位数组平均降低了0.062(95%CI:-0.116,-0.008)、6.429%(95%CI:-11.262%,-1.590%)、0.069(95%CI:-0.124,-0.014);与尿锑浓度第一分位数组相比,精子正常形态率(%)的自然对数值在第四分位数组平均降低了0.093(95%CI:-0.150,-0.035);与尿铅浓度第一分位数组相比,前向运动活力(%)(经平方根反正弦变换后)、精子总活力、精子存活率(%)(经平方根反正弦变换后)分别在第四分位数组平均降低了0.073(95%CI:-0.127,-0.020)、7.060%(95%CI:-11.876%,-2.244%)、0.085(95%CI:-0.140,-0.030);其余金属不同浓度四分位数分组之间的精液质量参数存在差异,但并无显著的剂量-反应关系(均P_(trend)>0.05)。结论尿液中锌和铅浓度与精子前向运动活力、精子总活力和精子存活率呈负相关,尿液中锑浓度与精子正常形态率呈负相关。
Objective To investigate the associations concentrations of 23types of urinary metals with semen quality parameters.Methods In this study,394men were recruited,and the semen and urine samples were collected.Inductively coupled plasma-mass spectrometry(ICP-MS)was used to detect the concentration of 23types of metal in the urine.After quartile grouping of each urine metal based on concentration,multiple linear regression analysis was used to explore the correlation between different concentrations of metals in urine and semen quality parameters.Results There was a significant dose-response relationship between urinary zinc,antimony,lead concentration and the semen quality parameters(P_(trend)<0.05).Compared with the 1st quartile group of urinary zinc concentration,progressive motility(%)(after square root arcsine transformed),total sperm motility and sperm vitality(%)(after square root arcsine transformed)were decreased by 0.062(95%CI:-0.116,-0.008),6.429%(95%CI:-11.262%,-1.590%)and 0.069(95%CI:-0.124,-0.014)in the 4th quartile group,respectively.Compared with the 1st quartile group of urinary antimony concentration,natural logarithmic value of normal sperm morphology rate(%)was decreased by 0.093(95%CI:-0.150,-0.035)on average in the 4th quartile group.Compared with the 1st quartile group of urinary lead concentration,progressive motility rate(%)(after square root arcsine transformed),total sperm motility and sperm vitality(%)(after square root arcsine transformed)were decreased by 0.073(95%CI:-0.127,-0.020),7.060%(95%CI:-11.876%,-2.244%)and 0.085(95%CI:-0.140,-0.030)in the 4th quartile group,respectively.The semen quality parameters of other metals in different quartiles were different,but there was no significant dose-response relationship(P_(trend)>0.05).Conclusion In this study,concentrations of zinc and lead in urine were negatively correlated with sperm motility,total sperm motility and sperm vitality.The concentration of antimony in urine was negatively correlated with normal sperm morphology rate.
