基于倾向评分匹配回顾性评价针刀治疗早、中期膝关节骨性关节炎

Retrospective Evaluation of Needle Knife Treatment of Knee Osteoarthritis at Early and Middle Stage Based on Propensity Score Matching

目的基于倾向评分匹配(PSM)观察针刀疗法对早、中期膝关节骨性关节炎(KOA)的临床疗效。方法分析2015年9月—2020年8月收治的早、中期膝关节骨性关节炎患者107例,其中观察组入组52例给予针刀疗法结合中药腾熨疗法、推拿点穴法治疗。对照组入组55例给予中药腾熨疗法和推拿点穴法治疗,运用PSM均衡组间基线后,以临床有效率和证候积分作为观察指标。经匹配,43对患者配对成功,两组涉及四个协变量分别为年龄、性别、病程和证候积分,通过匹配后均达到均衡(P>0.05)。通过两周治疗,观察临床有效率:观察组>对照组(95.35%VS 72.09%,P=0.003<0.05),观察证候积分发现:观察组的改善也好于对照组(P=0.003<0.05)。结论针刀疗法为早、中期膝关节骨性关节炎患者提供了又一治疗选择,值得推广。

Objective To observe the clinical efficacy of needle knife therapy on early and mid-stage of knee osteoarthritis(KOA)based on propensity score matching(PSM).Methods A total of 107 patients with early and mid-stage of KOA admitted from September 2015 to August 2020 were retrospectively analyzed and divided intothe observation group(52 cases)and the control group(55 cases).The observation group was treated with needle knife therapy combined with traditional Chinese medicine steaming and ironing therapy and Tuina,while the control group was treated with traditional Chinese medicine steaming and ironing therapy and Tuina.The clinical efficacyand evidence points of the two groups were observed after applying PSM to balance the baseline between groups.Results Forty-three pairs of patients were matched,and the four covariates of age,gender,duration of illness and symptoms score were balanced between the two groups(P>0.05).After 2 weeks of treatment,the observation group had a better overall effective rate than the control group(95.35%VS 72.09%,P=0.003<0.05),and the symptom scores of the observation group were also improved better than those of the control group(P=0.003<0.05).Conclusion Needle knife therapy provides another treatment option for patients with early and mid-stage of KOA and is worthy of clinical promotion.
