摘要
文章以1例老年脑卒中合并4期压疮患者为例,建立SBAR模式表,由传统口头交接班模式变换为SBAR标准化交接班模式,应用于科内护理交接班和三级医院-社区医院-居家护理三阶段的管理中,最终取得满意效果。
In this paper,a case of elderly stroke patients complicated with non-stage pressure ulcers was taken as an example.Through the establishment of SBAR model table,the traditional oral shift mode was transformed into SBAR standardized shift mode,which was applied to the management of in-department nursing shift and the three-level hospital-community hospital-home nursing,and finally achieved satisfactory results.
