三焦针法联合归脾汤加减治疗卒中后失眠心脾两虚证的临床疗效及对睡眠质量和生活质量的影响

Clinical efficacy of triple energizer acupuncture method combined with modified Guipi Decoction for post-stroke insomnia of heart-spleen deficiency and how to impact on sleep quality and quality-of-life

目的观察三焦针法联合归脾汤加减治疗卒中后失眠心脾两虚证的临床疗效及对患者睡眠质量及生活质量的影响。方法将2015年8月至2021年10月北京市怀柔区中医医院康复科84例卒中后失眠心脾两虚证患者,按照随机数字表法分为治疗组和对照组,各42例,最终2组各完成试验40例。2组患者均予卒中后常规治疗、基础康复训练及心理指导,对照组在此基础上予常规针刺治疗联合归脾汤加减,治疗组在对照组基础上予三焦针法治疗,2组疗程均为1个月。2组均采用匹兹堡睡眠质量指数量表(PSQI)评估患者睡眠质量,健康调查量表(SF-36)评估患者生活质量,并比较2组临床疗效。结果治疗组总有效率95.00%(38/40),对照组77.50%(31/40),治疗组疗效优于对照组(P<0.05)。2组治疗后PSQI和SF-36量表各子项评分均较本组治疗前改善(P<0.05),且治疗组治疗后改善程度优于对照组(P<0.05)。结论归脾汤联合三焦针法针刺可以显著改善卒中后失眠心脾两虚证患者睡眠状况,提高生活质量,临床疗效确切。

Objective To observe the clinical efficacy of triple energizer acupuncture method combined with modified Guipi Decoction for post-stroke insomnia(PSI)of heart-spleen deficiency and how to impact on sleep quality and quality-of-life.Methods A total of 84 PSI patients(heart-spleen deficiency)in the Department of Rehabilitation,Beijing Huairou Hospital of Traditional Chinese Medicine from August 2015 to October 2021 were randomly divided into the treatment group(n=42)and the control group(n=42),and finally 40 cases were completed in each group.All patients were managed routine treatment,basic rehabilitation training and psychological guidance;the control group was additionally treated with routine acupuncture treatment combined with modified Guipi Decoction,and the treatment group with triple energizer acupuncture method based on the control group.Treated for 1 month,sleep quality and quality-of-life was assessed by Pittsburgh sleep quality index scale(PSQI)and short-form 36 health survey scales(SF-36);the curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(95.00%[38/40]vs 77.50%[31/40],[P<0.05],respectively).After treatment,the PSQI scores and SF-36 scores in groups were better(P<0.05),the improvement of which in the treatment group were significantly better(P<0.05).Conclusion For PSI patients(heart-spleen deficiency),triple energizer acupuncture method combined with modified Guipi Decoction can significantly improve sleep status and quality-of-life,with definite clinical effect.
