温肾阳法对腰椎间盘突出症的疗效及对患者肌电图的影响

Efficacy of warming kidney yang method on lumbar disc herniation and its influence on electromyography of patients

目的探究温肾阳法治疗腰椎间盘突出症的临床疗效以及治疗前后肌电图运动神经传导速度(MCV)和运动单位电位(MUP)的变化。方法76例肝肾两虚型腰椎间盘突出症患者为研究对象,均进行右归丸方加减治疗及肌电图检查。观察76例患者中医治疗效果,对比治疗前后视觉模拟评分法(VAS)评分及MCV、MUP水平。结果76例患者中医治疗总有效率为84.2%。治疗后VAS评分(1.47±0.46)分低于治疗前的(5.03±1.40)分,差异有统计学意义(P<0.05)。治疗后治愈患者胫神经及腓深神经MCV、MUP时程均值及峰峰值均优于治疗前,差异均有统计学意义(P<0.05)。结论中医温肾阳法治疗腰椎间盘突出症疗效显著,治疗前后的肌电图变化对疗效评价具有指导意义,值得在临床推广应用。

Objective To investigate the clinical efficacy of warming kidney yang method in the treatment of lumbar disc herniation and the changes of electromyographic motor conduction velocity(MCV)and motor unit potential(MUP)before and after treatment.Methods 76 cases of lumbar disc herniation with deficiency of liver and kidney were treated with modified Yougui Pill and electromyography examination.The therapeutic effect of traditional Chinese medicine in 76 patients was observed.The visual analogue scale(VAS)score,MCV and MUP levels before and after treatment were compared.Results The total effective rate of traditional Chinese medicine treatment in 76 patients was 84.2%.The VAS score(1.47±0.46)points after treatment was lower than(5.03±1.40)points before treatment,and the difference was statistically significant(P<0.05).After treatment,the MCV of tibial nerve and deep peroneal nerve,mean time course and peak-to-peak value of MUP in cured patients were better than those before treatment,and the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine warming kidney yang method has significant efficacy in the treatment of lumbar disc herniation,and the electromyography changes before and after treatment have guiding significance for the evaluation of the effect,and it is worthy of clinical promotion and application.
