血清CYFRA21-1、HCY及CA15-3在乳腺癌患者中的表达水平及对乳腺癌的诊断价值分析

摘要 目的探讨血清细胞角蛋白19片段抗原(CYFRA21-1)、同型半胱氨酸(HCY)及糖类抗原15-3(CA15-3)在乳腺癌患者中的表达水平及对乳腺癌的诊断价值。方法选取113例乳腺癌患者作为观察组,60例良性乳腺疾病患者作为对照1组,100例同期体检健康者作为对照2组。检测并对比三组血清CYFRA21-1、HCY、CA15-3水平,并分析CYFRA21-1、HCY、CA15-3单项及联合检测对乳腺癌的诊断价值。结果三组的CYFRA21-1、HCY、CA15-3水平对比差异具有统计学意义(P<0.05);观察组的血清CYFRA21-1(7.74±4.87)ng/ml、HCY(14.30±4.88)μmol/L、CA15-3(45.84±18.87)U/ml均高于对照1组的(2.51±0.81)ng/ml、(7.40±2.38)μmol/L、(13.43±4.12)U/ml和对照2组的(2.54±0.82)ng/ml、(7.85±2.68)μmol/L、(9.63±3.20)U/ml,对照1组的CA15-3水平高于对照2组,差异具有统计学意义(P<0.05);对照1组和对照2组的血清CYFRA21-1、HCY水平对比差异无统计学意义(P>0.05)。三项联合检测诊断乳腺癌的灵敏度、准确度均高于单项CYFRA21-1、HCY、CA15-3检测,CA15-3检测诊断乳腺癌的灵敏度、准确度均高于CYFRA21-1、HCY检测,差异具有统计学意义(P<0.05);三项联合检测诊断乳腺癌的特异度86.67%(52/60)高于单项CA15-3检测的71.67%(43/60),差异具有统计学意义(P<0.05)。结论血清CYFRA21-1、HCY、CA15-3在乳腺癌中呈现显著高水平,联合检测诊断的总体价值较高。 Objective To discuss the expression levels of serum cytokeratin 19 fragment(CYFRA21-1),homocysteine(HCY)and carbohydrate antigen 15-3(CA15-3)in breast cancer patients and their diagnostic value for breast cancer.Methods A total of 113 patients with breast cancer were selected as the observation group,60 patients with benign breast disease were selected as the control group 1,and 100 healthy subjects during the same period were selected as the control group 2.The serum levels of CYFRA21-1,HCY and CA15-3 in the three groups were detected and compared,and the diagnostic value of separate and combined detection of CYFRA21-1,HCY and CA15-3 for breast cancer was analyzed.Results There were statistically significant differences in the levels of CYFRA21-1,HCY and CA15-3 among the three groups(P<0.05).The serum CYFRA21-1(7.74±4.87)ng/ml,HCY(14.30±4.88)μmol/L,CA15-3(45.84±18.87)U/ml in observation group were higher than those in control group 1[(2.51±0.81)ng/ml,(7.40±2.38)μmol/L,(13.43±4.12)U/ml]and control group 2[(2.54±0.82)ng/ml,(7.85±2.68)μmol/L,(9.63±3.20)U/ml];the CA15-3 in control group 1 was higher than that in control group 2;all the differences were statistically significant(P<0.05).There was no statistically significant difference in serum CYFRA21-1 and HCY levels between control group 1 and control group 2(P>0.05).The sensitivity and accuracy of the combined detection in diagnosing breast cancer were higher than those of CYFRA21-1,HCY,and CA15-3 detection alone,and the differences were statistically significant(P<0.05).The specificity of combined detection for the diagnosis of breast cancer was 86.67%(52/60),which was higher than 71.67%(43/60)of CA15-3 detection,and the difference was statistically significant(P<0.05).Conclusion The levels of serum CYFRA21-1,HCY and CA15-3 are significantly high in breast cancers,and the overall value of combined detection and diagnosis is high.
