探讨替米沙坦联合胺碘酮治疗高血压伴阵发性心房颤动患者的临床效果

Clinical effect of telmisartan combined with amiodarone in the treatment of hypertensive patients with aroxysmal atrial fibrillation

目的探讨替米沙坦联合胺碘酮治疗高血压伴阵发性心房颤动患者的临床效果。方法80例高血压伴阵发性心房颤动患者,随机分为对照组和研究组,各40例。对照组采用胺碘酮治疗,研究组采用替米沙坦联合胺碘酮治疗。比较两组治疗效果,左心室舒张末期内径、左心室收缩末期内径,治疗前后心房颤动发作次数、发作持续时间,6个月及1年时复发率。结果研究组总有效率97.5%高于对照组的75.0%,差异具有统计学意义(P<0.05)。治疗后,两组心房颤动发作次数、发作持续时间均优于治疗前,且研究组心房颤动发作次数、发作持续时间均优于对照组,差异具有统计学意义(P<0.05)。研究组左心室舒张末期内径(34.3±3.6)mm及左心室收缩末期容积(24.5±3.0)mm均小于对照组的(40.1±3.3)、(29.6±4.4)mm,差异具有统计学意义(P<0.05)。6个月及1年时,研究组复发率分别为12.5%、17.5%均低于对照组的32.5%、42.5%,差异具有统计学意义(P<0.05)。结论将替米沙坦联合胺碘酮用于高血压伴阵发性心房颤动患者的治疗中,患者心房颤动复发率有极大降低,临床疗效明显提高,心功能也有显著改善,值得临床推广。

Objective To discuss the clinical effect of telmisartan combined with amiodarone in the treatment of hypertensive patients with aroxysmal atrial fibrillation.Methods A total of 80 patients with hypertension and paroxysmal atrial fibrillation were randomly divided into control group and research group,with 40 cases in each group.The control group was treated with amiodarone,and the research group was treated with telmisartan and amiodarone.Both groups were compared in terms of therapeutic effect,left ventricular end-diastolic diameter,left ventricular end-systolic diameter,frequency and duration of atrial fibrillation before and after treatment,and the recurrence rate at 6 months and 1 year.Results The total effective rate 97.5%of the research group was higher than 75.0%of the control group,and the difference was statistically significant(P<0.05).After treatment,the frequency and duration of atrial fibrillation in the two groups were better than those before treatment in this group,and the frequency and duration of atrial fibrillation in the research group were better than those in the control group.All the differences were statistically significant(P<0.05).The left ventricular end-diastolic diameter(34.3±3.6)mm and the left ventricular end-systolic volume(24.5±3.0)mm in the research group were less than(40.1±3.3)and(29.6±4.4)mm in the control group,and the differences were statistically significant(P<0.05).At 6 months and 1 year,the recurrence rates of the research group were 12.5%and 17.5%,which were lower than 32.5%and 42.5%of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of telmisartan combined with amiodarone in the treatment of hypertensive patients with paroxysonal atrial fibrillation can significantly reduce the recurrence rate of atrial fibrillation,significantly improve the clinical efficacy and cardiac function of patients,which is worthy of clinical promotion.
