摘要
目的探讨育龄期子宫内膜息肉与阴道微生态平衡的关系。方法收集我院收治的92例育龄期子宫内膜息肉患者的临床资料,纳入子宫内膜息肉组,同时收集我院同期91例健康体检育龄期妇女的临床资料,纳入对照组。记录患者阴道微生态指标,分析阴道微生态平衡与子宫内膜息肉的关系,并检验阴道微生态指标对子宫内膜息肉发生风险的预测价值。结果经秩和或χ^(2)检验发现,白细胞酯酶、唾液酸苷酶、乙酰氨基葡萄糖苷酶、清洁度、上皮细胞、白细胞、线索细胞、短杆菌及阴道微生态菌群异常可能是子宫内膜息肉发生的相关因素(P<0.05);经二元Logistic回归分析发现,白细胞、白细胞酯酶及阴道微生态菌群异常均是子宫内膜息肉发生的独立危险因素(P<0.05);绘制受试者工作特征(ROC)曲线发现,白细胞+白细胞酯酶异常、白细胞酯酶+阴道微生态菌群异常、白细胞+白细胞酯酶+阴道微生态菌群异常预测子宫内膜息肉发生风险的曲线下面积(AUC)分别为0.782、0.737、0.808,对子宫内膜息肉发生风险有中等预测效能。结论育龄期子宫内膜息肉的发生与阴道微生态平衡可能存在相关性,提前干预异常的阴道微生态可以预防子宫内膜息肉的发生。
Objective To investigate the relationship between endometrial polyps and vaginal micro-ecological balance in childbearing age.Methods The clinical date of 92 patients with endometrial polyps in childbearing age treated in our hospital were collected and included in the endometrial polyp group.At the same time,the clinical data of 91 women with health check-up in childbearing age in our hospital during the same period were collected and included in the control group.The vaginal micro-ecological indicators of patients were recorded,the relationship between vaginal micro-ecological balance and endometrial polyps was analyzed,and the predictive value of vaginal micro-ecological indicators for the risk of endometrial polyps was tested.Results Rank sum orχ^(2) test revealed that the abnormality of leukocyte lipase,sialidase,acetylglucosaminidase,cleanliness,epithelial cells,leukocytes,clue cells,brevibacterium and vaginal micro-ecological flora may be the related factors for the occurrence of endometrial polyps(P<0.05).Binary Logistic regression analysis show that abnormal leukocytes,leukocyte lipase and vaginal micro-ecological flora were the independent risk factors for the occurrence of endometrial polyps(P<0.05).The receiver operating characteristic(ROC)curve was drawn to find that the area under the curve(AUC)of abnormal leukocyte+leukocyte lipase,abnormal leukocyte lipase+vaginal micro-ecological flora and abnormal leukocyte+leukocyte lipase+vaginal micro-ecological flora for predicting the risk of endometrial polyps were 0.782,0.737,0.808,respectively,with moderate predictive effect for the risk of endometrial polyps.Conclusion The occurrence of endometrial polyps in childbearing age may be related to the vaginal micro-ecological balance.Early intervention of abnormal vaginal micro-ecology can prevent the occurrence of endometrial polyps.
