脑卒中后抑郁患者睡眠主观评估和多导睡眠监测的差异

Differences Between Subjective Sleep Assessment and Polysomnography Monitoring in Patients with Post-stroke Depression

目的探讨脑卒中后抑郁患者主观睡眠与客观睡眠的差异,为指导患者的睡眠提供临床依据。方法选择2017年10月至2019年10月于郑州颐和医院住院的78例脑卒中后抑郁患者。采用匹兹堡睡眠质量指数(PSQI)和简式睡眠信念和态度量表(DBAS)评定主观睡眠质量。使用整夜多导睡眠监测仪进行客观睡眠监测。分析主观睡眠质量与客观睡眠质量之间的差异,并采用Pearson相关系数分析主客观睡眠质量与各因子之间的相关性。结果与多导睡眠监测得出的数据比较,借助PSQI得出的睡眠时间较短,睡眠潜伏期较长,睡眠效率较低(P<0.05)。DBAS担忧评分与多导睡眠监测中睡眠Ⅰ期时间、微觉醒呈正相关(P<0.05);DBAS后果评分与睡眠Ⅲ期时间呈正相关(P<0.05)。结论脑卒中后抑郁患者的主观睡眠质量差于客观睡眠质量。错误的睡眠信念和态度会对患者的睡眠质量产生影响。

Objective To explore the difference between subjective sleep and objective sleep in patients with post-stroke depression,and to provide a clinical basis for guiding the sleep of patients.Methods A total of 78 patients with post-stroke depression hospitalized in Zhengzhou Yihe Hospital from October 2017 to October 2019 were selected.Pittsburgh sleep quality index(PSQI)and brief version of dysfunctional beliefs and attitudes about sleep(DBAS)was used to evaluate subjective sleep quality.Objective sleep quality was carried out by using an all-night polysomnography monitor.The difference between subjective sleep quality and objective sleep quality was analyzed,and Pearson correlation coefficient was used to analyze the correlation between subjective and objective sleep quality and various factors.Results Compared with the data obtained from polysomnography,the results obtained by PSQI showed shorter sleep time,longer sleep latency and lower sleep efficiency(P<0.05).The score of worry in DBAS was positively correlated with the time of phase Ⅰ and micro-wakefulness in polysomnography monitoring(P<0.05).The score of consequence in DBAS was positively correlated with the time of stage Ⅲ(P<0.05).Conclusion The subjective sleep quality of patients with post-stroke depression is worse than the objective sleep quality,and the faulty sleep belief and attitude can adversely affect the quality of patients’sleep.
