斜仰卧截石位在复杂性肾结石患者术中的应用效果

Application Effect of Oblique Supine Lithotomy Position in the Operation of Patients with Complex Kidney Stones

目的观察斜仰卧截石位在复杂性肾结石患者术中的应用效果。方法选取2020年1月至2021年12月黄河水利委员会黄河中心医院收治的88例复杂性肾结石患者。按照手术室护理体位不同分为对照组(44例)和观察组(44例)。对照组患者在手术室采用俯卧位;观察组患者在手术室采用斜仰卧截石位。比较两组舒适度、术中各项生命体征情况(收缩压、舒张压、血氧饱和度、心率)、康复进程(手术时间、体位摆放时间、住院时间)、体位相关并发症的发生情况。结果观察组患者舒适度高于对照组(P<0.05)。体位摆放时和手术结束时,两组舒张压、收缩压、血氧饱和度、心率比较,差异无统计学意义(P>0.05)。观察组体位摆放时间、手术时间、住院时间短于对照组(P<0.05)。观察组体位相关并发症发生率低于对照组(P<0.05)。结论复杂性肾结石患者术中采用斜仰卧截石位不仅能降低体位相关并发症的发生风险,加快体位摆放速度,而且能提高患者舒适度,促进患者康复。

Objective To observe the effect of the oblique supine lithotomy position in the operation of patients with complex kidney stones.Methods A total of 88 patients with complex kidney stones who were admitted to the Yellow River Central Hospital of the Yellow River Water Conservancy Commission from January 2020 to December 2021 were selected.They were divided into control group(44 cases)and observation group(44 cases)according to different nursing positions in the operating room.The patients in control group were given the prone position nursing in the operating room,and the patients in observation group were given the oblique supine lithotomy position nursing in the operating room.The degree of comfort,intraoperative vital signs(systolic blood pressure,diastolic blood pressure,blood oxygen saturation,heart rate),rehabilitation process(operation time,posture placement time,hospitalization time),and the incidence of complications related to the position were compared between the two groups.Results The comfort level of patients in observation group was higher than that in control group(P<0.05).There was no statistical difference in diastolic blood pressure,systolic blood pressure,blood oxygen saturation and heart rate between the two groups at the time of posture placement and at the end of operation(P>0.05).The time of posture placement,operation and hospitalization in observation group were shorter than those in control group(P<0.05).The incidence of posture-related complications in observation group was lower than that in control group(P<0.05).Conclusion Oblique supine lithotomy can not only reduce the risk of posture-related complications,but also speed up posture placement in patients with complicated kidney calculi,and improve the comfort of patients and promote their recovery.
