奥司他韦联合羚羊清肺散治疗儿童流感病毒肺炎的效果

Effects of Oseltamivir Combined with Lingyang Qingfei Powder in the Treatment of Children with Influenza Virus Pneumonia

目的探讨奥司他韦联合羚羊清肺散对流感病毒肺炎(IVP)患儿的疗效。方法选取2019年8月至2021年6月西平县人民医院收治的104例IVP患儿。按随机数表法将患儿分为常规组(52例)和联合组(52例)。患儿均接受基础治疗。给予常规组奥司他韦治疗。在常规组基础上,给予联合组羚羊清肺散治疗。治疗1周后,观察两组疗效、症状体征改善时间及不良反应情况。比较两组治疗前后呼气峰值流速(PEF)、第1秒用力呼气容积(FEV_(1))、细胞免疫功能(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及血清炎症介质水平,包括几丁质酶样蛋白(YKL-40)、表面活性蛋白D(SFTPD)。结果联合组治疗总有效率较常规组高,症状体征改善时间均短于常规组(P<0.05)。两组治疗后PEF、FEV_(1)升高,联合组治疗后PEF、FEV_(1)较常规组高(P<0.05)。治疗后,两组CD4^(+)、CD4^(+)/CD8^(+)升高,联合组CD4^(+)、CD4^(+)/CD8^(+)较常规组高(P<0.05);治疗后,两组CD8^(+)降低,联合组CD8^(+)较常规组低(P<0.05)。两组治疗后血清YKL-40、SFTPD水平降低,联合组治疗后血清YKL-40、SFTPD水平较常规组低(P<0.05)。联合组不良反应发生率与常规组比较,差异无统计学意义(P>0.05)。结论奥司他韦联合羚羊清肺散治疗IVP患儿的效果显著,能减轻炎症发应,有助于改善免疫功能及肺功能,缓解症状。

Objective To investigate the effect of oseltamivir combined with Lingyang Qingfei powder in the treatment of children with influenza virus pneumonia(IVP).Methods A total of 104 children with IVP in Xiping County People’s Hospital from August 2019 to June 2021 were selected.According to the random number table method,the children were divided into conventional group(52 cases)and combined group(52 cases).All children received basic treatment.The conventional group was given oseltamivir treatment.On the basis of the conventional group,the combined group was given Lingyang Qingfei powder for treatment.After 1 week of treatment,the curative effect,symptoms and signs improvement time and adverse reactions of the two groups were observed.The peak expiratory flow rate(PEF),forced expiratory volume in 1 second(FEV_(1)),cellular immune function(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and serum levels of inflammatory mediators,including chitinase-like protein(YKL-40)and surfactant protein D(SFTPD)were compared between the two groups before and after treatment.Results The total effective rate of treatment in combined group was higher than that in conventional group,and the improvement time of symptoms and signs was shorter than that in conventional group(P<0.05).After treatment,PEF and FEV_(1) in the two groups increased,and PEF and FEV_(1) in combined group were higher than those in conventional group after treatment(P<0.05).After treatment,CD4^(+) and CD4^(+)/CD8^(+) in the two groups increased,and CD4^(+)and CD4^(+)/CD8^(+) in combined group were higher than those in conventional group(P<0.05).After treatment,CD8^(+) in the two groups decreased,and CD8^(+) in combined group was lower than that in conventional group(P<0.05).Serum YKL-40 and SFTPD levels in the two groups decreased after treatment,and serum YKL-40 and SFTPD levels in combined group were lower than those in conventional group after treatment(P<0.05).There was no statistical difference in the incidence of adverse reactions between combined group and conventional group(P>0.05).Conclusion Oseltamivir combined with Lingyang Qingfei powder is effective in the treatment of children with IVP,which can reduce inflammation,improve immune function and lung function,and relieve symptoms.
