自制痔疮膏联合腹针疗法预防性治疗混合痔术后并发症的临床研究

Clinical Study of Self-made Hemorrhoid Ointment Combined with Abdominal Acupuncture for Preventive Treatment of Postoperative Complications of Mixed Hemorrhoids

目的:观察自制痔疮膏联合腹针疗法预防性治疗混合痔外剥内扎术后并发症的临床效果。方法:选取2019年6月至2021年2月南昌市洪都中医院确诊为混合痔的患者120例,按照随机数字表法分为对照组和观察组,每组60例。两组患者在骶管麻醉下行混合痔外剥内扎术,对照组在术后给予凡士林外敷,观察组自制痔疮膏联合腹针疗法治疗。观察两组并发症发生情况、并发症消失时间及临床疗效。结果:两组治疗前便血、排便困难、尿潴留、疼痛发生人数的差异无统计学意义(P>0.05),治疗后两组便血、排便困难、尿潴留、疼痛发生人数均显著降低,且与对照组比较,观察组降低更多,差异有统计学意义(P<0.05)。治疗后观察组便血消失时间、排便困难消失时间、尿潴留消失时间、疼痛消失时间均短于对照组,差异有统计学意义(P<0.05)。观察组有效率高达98.33%,显著高于对照组80%有效率(P<0.01)。结论:自制痔疮膏联合腹针疗法预防性治疗混合痔术后并发症的临床疗效显著。

Objective:To explore the clinical study of self-made hemorrhoid ointment combined with abdominal acupuncture in the preventive treatment of postoperative complications of mixed hemorrhoids.Methods:120 patients diagnosed with mixed hemorrhoids in Hongdu Hospital of Traditional Chinese Medicine,Nanchang City,were selected and divided into control group and observation group according to random number table method,60 cases in each group.Two groups of patients underwent sacral anesthesia with mixed hemorrhoids external resection and internal ligation,the control group was given external application of petroleum jelly after the operation,and the observation group was treated with homemade hemorrhoid ointment combined with abdominal acupuncture.The occurrence of complications,disappearance time and clinical efficacy of the two groups were observed.Results:There was no statistically significant difference in the number of blood in the stool,dysfunction,urinary retention,and pain between the two groups before treatment(P>0.05).After treatment,the number of blood in the stool,dysfunction,urinary retention,and pain in the two groups was significantly reduced,and compared with the control group,the observation group decreased more,and the difference was statistically significant(P<0.05).After treatment,the disappearance time of blood in the stool,the disappearance time of defecation difficulty,the disappearance time of urinary retention,and the disappearance time of pain in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The effective rate of the observation group was as high as 98.33%,which was significantly higher than the effective rate of 80.00%of the control group,and the difference was statistically significant(P<0.01).Conclusion:The self-made hemorrhoid ointment combined with abdominal acupuncture has a significant clinical effect in the preventive treatment of postoperative complications of mixed hemorrhoids.
