黄帝内针联合关节松动术治疗中风后偏瘫侧肩痛的临床疗效

Clinical Effect of Huangdi Internal Acupuncture Combined with Joint Mobilization in the Treatment of Hemiplegic Shoulder Pain after Stroke

目的:观察黄帝内针联合关节松动术治疗中风后偏瘫侧肩痛的临床疗效。方法:将56例符合纳入标准的患者随机分为观察组28例和对照组28例,两组予西药+关节松动术治疗作为常规基础治疗,观察组予常规基础治疗上予黄帝内针治疗。两组均治疗14d后,观察两组治疗前后视觉模拟量表(Visual Analogue Scale,VAS)评分、肢体运动功能Fugl-Meyer评定量表(Fugl-Meyer assessment,FMA)评分、日常生活能力评定量表(Activity of Daily Living,ADL)评分的变化,并比较两组疼痛程度。结果:两组治疗后与同组治疗前比较VAS评分降低(P<0.01),上肢运动功能FMA评分及ADL评分增加(P<0.01),且观察组治疗后.上肢运动功能FMA评分及ADL评分高于对照组治疗后(P<0.05),观察组治疗后VAS评分低于对照组治疗后(P<0.01),观察组疼痛程度明显轻于对照组(P<0.01)。结论:黄帝内针联合关节松动术治疗中风后偏瘫侧肩痛效果确切,可改善上肢运动功能,能提高日常生活能力、缓解肩关节疼痛及降低患者疼痛程度。

Objective: To observe the clinical effect of Huangdi internal acupuncture combined with joint mobilization in the treatment of hemiplegic shoulder pain after stroke. Methods: 56 patients who met the inclusion criteria were randomly divided into observation group(28 cases) and control group(28 cases). The two groups were treated with western medicine and joint mobilization as routine basic treatment. The bservation group was treated with Huangdi internal acupuncture on the basis of routine treatment. After 14 days of treatment in both groups, the changes of Visual Analogue Scale(VAS)score, Fugl-Meyer Assessment(FMA) score, Activity of Daily Living scale(ADL) score were observed before and after treatment, and the the pain degree of the two groups was compared. Results: After treatment,compared with the same group before treatment, the VAS score of the two groups decreased(P < 0.01), the FMA score and ADL score of upper limb motor function increased(P < 0.01), and the FMA score and ADL score of upper limb motor function in the observation group after treatment were higher than those in the control group after treatment(P < 0.05), the VAS score of the observation group after treatment was lower than that in the control group after treatment(P < 0.01), and the degree of pain in the observation group was significantly lighter than that in the control group(P < 0.01). Conclusion: Huangdi internal acupuncture combined with joint mobilization is effective in the treatment of hemiplegic shoulder pain after stroke, it can improve the motor function of upper limbs, improve the ability of daily living,relieve shoulder pain and reduce the pain degree of patients.
