硬下疳样生殖器疱疹合并HIV感染1例7年随访报告

Hard Chancre-like Herpes Simplex Infection Combined HIV:A Case Report and 7-year Follow-up Study

患者男,66岁,阴茎破溃伴疼痛2周,阴茎可见2个硬下疳样溃疡。梅毒血清学阴性,分泌物HSV2-DNA 2.69×10^(5)拷贝/mL。诊断:生殖器疱疹合并HIV感染。予伐昔洛韦口服1个月后皮损愈合。至今随访7年余,期间多次尝试停药,每次停药2~3 d皮损便复发,故现仍坚持服药。

A 66-year-old male,HIV-positive,presented to our department with a 2-week history of an extensive and painful genital ulceration.Two hard chancre-like ulcerations were seen on his penis.Serological tests for syphilis were negative,while HSV_(2)-DNA test was positive.He was diagnosed as genital herpes with HIV.Oral valaciclovir was started immediately,which led to progressive and complete response in about 1 month.Until now,he has been taking the medicine discontinuously for more than 7 years.During the years,he tried many times to stop taking the drug,but all experienced recurrences within 2 to 3 days.
